目的：探讨乳腺非肿块型强化病变的 MRI 影像学特点。方法回顾性分析54例有病理结果的非肿块病灶的 MRI 主要影像特征。结果病理结果19例为恶性，35例为良性。平扫病灶的形态和信号特征不明确。增强扫描：恶性组，病灶分布呈节段性和区域性不均匀强化的占47．3％（9／19）；良性组，病灶呈网状或点簇状均匀性强化的占28．6％（10／35）。MRI 诊断乳腺非肿块样病变的敏感度、特异度分别为90．7％、61．4％。结论MRI 对乳腺非肿块型强化病变具有较高的诊断价值，结合其形态学特征及 DWI 有助于非肿块样强化病变的鉴别诊断。

Objective To explore the MRI image manifestations of breast diseases presented as non-mass like enhancement(NMLE).Methods 54 cases presented as NMLE lesions which confirmed by pathological examination were analyzed retrospectively.All the pathologic results and imaging features were assessed accord-ingly.Results Among the 54 cases of NMLE,19 cases were malignant tumors and 35 cases were benign mas-ses.The morphological features and MRI signal characteristics of these lesions are not in accordance with those in non-enhanced MRI scan.After injecting the contrast agent,some lesions showed different tissue enhancement modes.In malignant group,9 /19 cases showed segmental and regional inhomogeneous enhancement which ac-counting for 47.3%;while in benign group,assess with focal net-like or point clusters homogeneous enhance-ment accounted for 28.6%(10 /35).The sensitivity and specificity of MR diagnostic is 90.7% and 61.4% re-spectively.Conclusions MRI examination is of great value in NMLE diagnosis in female breast diseases,com-bination of morphological characteristics and DWI are helpful in differential diagnosis.
