目的检测急性心肌梗死早期(发病到就诊时间<2h)患者血清中的蛋白表达谱,筛选并鉴定出特异的标志蛋白。方法选取20例急性心肌梗死早期患者(早期急性心肌梗死组)和40例稳定性心绞痛患者(稳定性心绞痛组),采用激光解析电离飞行时间质谱技术,检测两组患者血清中的蛋白峰强度,寻找差异表达蛋白并建立蛋白分类模式,确定诊断的特异度和敏感度。采用液相质谱技术进一步鉴定已筛选出来的蛋白。结果早期急性心肌梗死组有5个蛋白显著高表达,质荷比分别为1530.32、4301.46、8683.44、23386.1、23625.5Da,与稳定性心绞痛组患者比较,差异有统计学意义(P<0.01)。建立决策树,早期诊断急性心肌梗死的敏感度为93.33﹪,特异度为85.00%,阳性预测值为93.33%。经过鉴定,phosphoglycerate mutase 1在急性心肌梗死早期患者中高表达。结论通过蛋白质组学技术可以早期发现急性心肌梗死,phosphoglycerate mutase 1为早期特异表达蛋白。

Objective To screen early serum protein markers for acute myocardial infarction by SELDI- TOF mass spec-trometry. Methods Twenty patients with acute myocardial infarction admitted within 2h after onset of symptoms, and 40 patients with stable angina pectoris were enrol ed in this study. Serum protein profile was analyzed by surface enhanced laser desorption ionization- time of flight- mass spectrometry (SELDI- TOF- MS). The differential y expressed proteins were identified by high per-formance 1iquid chromatography- electrospray ionization mass spectrometry (LC- MS). Results Compared to stable angina pectoris, 5 proteins were significantly over- expressed in acute myocardial infarction(P<0.01): 1 530.32, 4 301.46, 8 683.44, 23 386.1, 23 625.5Da. Based on differential y expressed proteins a diagnosis model for acute myocardial infarction was con-structed. The sensitivity, specificity and positive predictive value of this model were 93.33%, 85%and 93.33%, respectively. A-mong 5 differential y expressed proteins phosphoglycerate mutase 1 was identified, which was over- expressed in patients with early stage myocardial infarction. Conclusion Some serum proteins are over- expressed in patients with acute myocardial in-farction including phosphoglycerate mutase 1.
