目的评价股动脉-股深动脉人造血管动脉搭桥在血液透析中的有效性。方法选择18例行股动脉-股深动脉人造血管搭桥术(A组)、20例自体动静脉内瘘(B组)及25例半永久性血液透析通路(双腔带Cuff导管)(C组)患者,随访并分析比较3组患者术后3、6个月及1年感染率、术后通畅率、血白蛋白、血红蛋白、C反应蛋白、尿素清除指数(Kt/v)值。结果 A组与B、C组比较,3个时点感染率、通畅率、血白蛋白、血红蛋白、Kt/v值的差异均无统计学意义(均P>0.05),但C反应蛋白的差异有统计学意义(P<0.05)。结论短期内通畅率、感染率与自体动静脉内瘘、半永久性血液透析通路无统计学差异;股动脉-股深动脉人造血管搭桥术后血白蛋白、血红蛋白、Kt/v值与自体动静脉内瘘、半永久性血液透析通路无统计学差异。

목적평개고동맥-고심동맥인조혈관동맥탑교재혈액투석중적유효성。방법선택18례행고동맥-고심동맥인조혈관탑교술(A조)、20례자체동정맥내루(B조)급25례반영구성혈액투석통로(쌍강대Cuff도관)(C조)환자,수방병분석비교3조환자술후3、6개월급1년감염솔、술후통창솔、혈백단백、혈홍단백、C반응단백、뇨소청제지수(Kt/v)치。결과 A조여B、C조비교,3개시점감염솔、통창솔、혈백단백、혈홍단백、Kt/v치적차이균무통계학의의(균P>0.05),단C반응단백적차이유통계학의의(P<0.05)。결론단기내통창솔、감염솔여자체동정맥내루、반영구성혈액투석통로무통계학차이;고동맥-고심동맥인조혈관탑교술후혈백단백、혈홍단백、Kt/v치여자체동정맥내루、반영구성혈액투석통로무통계학차이。
Objective To compare different types of access in hemodialysis. Methods Sixty three patients underwent hemodialysis with different access:in group A (n=18) artificial blood vessel graft between femoral artery and deep femoral artery was constructed, in group B(n=20) arteriovenous fistula was created and in group C(n=25) the tunneled central venous catheters were applied. The infection rate and patency rate were assessed;and albumin, hemoglobin, CRP and Urea clearance index (kt/v) were measured at 3, 6 and 12 months after operation. Results There were no significant differences in infection rate, patency rate, albumin, hemoglobin, urea clearance index in al time points among three groups;however, compared to group B and C the CRP level in group A was higher during the fol ow- up period. Conclusion Compare to arteriovenous fistula and tunneled central venous catheters, there is no difference in effectiveness and blood biochemical parameters for the access of artificial blood ves-sel graft between femoral artery and deep femoral artery in hemodialysis.
