目的：对经尿道前列腺电切术（TURP）后发生膀胱颈挛缩（BNC）的各种因素进行多元回归分析，探寻主要影响因素。方法对2009年1月至2013年4月在我院泌尿外科接受TURP术治疗的前列腺增生患者812例进行随访，发现TURP术后的BNC患者，并选择同期内在我院泌尿外科行TURP术，术后1年内未出现BNC的患者进行配对，进行条件logistic回归分析。结果术前前列腺重量、单位时间切除重量、电凝切开深度这3个因素进入到多元回归方程，OR值分别为2．519、3．374、1．420。结论术前前列腺重量、单位时间切除重量、电凝切开深度是影响TURP术后BNC发生的主要因素，临床中应重视这些风险因素的参考价值，提高术者的手术操作技巧、术后对患者密切随访和及时处理是降低这种并发症的有效措施。
목적：대경뇨도전렬선전절술（TURP）후발생방광경련축（BNC）적각충인소진행다원회귀분석，탐심주요영향인소。방법대2009년1월지2013년4월재아원비뇨외과접수TURP술치료적전렬선증생환자812례진행수방，발현TURP술후적BNC환자，병선택동기내재아원비뇨외과행TURP술，술후1년내미출현BNC적환자진행배대，진행조건logistic회귀분석。결과술전전렬선중량、단위시간절제중량、전응절개심도저3개인소진입도다원회귀방정，OR치분별위2．519、3．374、1．420。결론술전전렬선중량、단위시간절제중량、전응절개심도시영향TURP술후BNC발생적주요인소，림상중응중시저사풍험인소적삼고개치，제고술자적수술조작기교、술후대환자밀절수방화급시처리시강저저충병발증적유효조시。
Objective To analyze various factors related to the occurrence of bladder neck contractures (BNC) after transurethral resection of prostate (TURP) ,and to explore the main influencing factors .Methods A total of 812 benign prostatic hyperplasia (BPH) patients who underwent TURP in our hospital during Jan .2009 to April 2013 were followed up for 12 months .Patients who developed BNC patients were selected .At the same time ,none BNC patients after TURP were also selected .Conditional logistic regression was used to analyze the major influencing factors .Results Three factors ,including volume of the prostate ,volume of resected prostate ,and the incision depth were the major influencing factors ,and the OR was 2 .519 ,3 .374 ,and 1 .420 ,respectively .Conclusion Volume of the prostate ,volume of resected prostate ,the incision depth were the major influencing factors of BNC ,which should be paid due attention .Improved operation skills and close followed‐up are important in reducing the morbidity of BNC .
