目的:应用二尖瓣环自动追踪技术(AMAT)定量检测慢性肾功能衰竭(CRF)患者二尖瓣环运动参数,评价左心室收缩功能。方法尿毒症组18例,非尿毒症组16例,正常对照组22例,取心尖四腔心切面,运用 AMAT 技术测量二尖瓣环左室间隔侧、侧壁纵向位移(Y-Posi L、Y-Posi R)、横向位移(X-Posi L、X-Posi R)等参数,并进行统计分析。结果与对照组比较,尿毒症组左心室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、射血分数(LVEF)显著降低,X-Posi L、Y-Posi L 显著降低,非尿毒症组 X-Posi L 亦显著降低。纳入病例 X-Posi L、Y-Posi L 与 LVEF 均呈显著正相关(r 分别为0.67、0.71,P <0.05)。结论AMAT 可以评价CRF 患者患者左心室收缩功能。

목적:응용이첨판배자동추종기술(AMAT)정량검측만성신공능쇠갈(CRF)환자이첨판배운동삼수,평개좌심실수축공능。방법뇨독증조18례,비뇨독증조16례,정상대조조22례,취심첨사강심절면,운용 AMAT 기술측량이첨판배좌실간격측、측벽종향위이(Y-Posi L、Y-Posi R)、횡향위이(X-Posi L、X-Posi R)등삼수,병진행통계분석。결과여대조조비교,뇨독증조좌심실서장말기용적(LVEDV)、수축말기용적(LVESV)、사혈분수(LVEF)현저강저,X-Posi L、Y-Posi L 현저강저,비뇨독증조 X-Posi L 역현저강저。납입병례 X-Posi L、Y-Posi L 여 LVEF 균정현저정상관(r 분별위0.67、0.71,P <0.05)。결론AMAT 가이평개CRF 환자환자좌심실수축공능。
Objective To explore the value of mitral annular displace based on AMAT in evaluating left ventricular function in pa-tients with chronic renal failure.Methods There were 34 chronic renal failure cases in groups(uremia group 18 cases,non-uremia group 16 cases),and 22 cases in normal control group.Apical four chamber view was acquired,and AMAT was used to measure left ventricular septal and lateral wall mitral annular longitudinal and horizontal displacement(Y-Posi L,Y-Posi R,X-Posi L,X-Posi R),then statistical analysis was performed.Results Compared with the control group,LVEDV,LVESV,LVEF decreased significantly in uremia group,X-Posi L,Y-Posi L al-so decreased significantly.X-Posi L decreased significantly in non-uremia group.X-Posi L,Y-Posi L were positively correlated with LVEF in all cases(r =0.67,0.71,P <0.05).Conclusion AMAT can evaluate the left ventricular systolic function in patients with chronic renal failure.
