目的:观察经鼻气管插管与经口气管插管对严重衰竭患者的临床效果。方法100例Ⅱ型呼吸衰竭需要气管插管的患者,随机分为两组,每组50例,甲组采用纤维支气管镜引导下经鼻气管插管方式建立人工气道,乙组采用喉镜引导下经口气管插管方式,对比两组血气分析指标、生命体征变化及插管情况。结果甲组生命体征变化情况优于乙组,差异有统计学意义(P<0.05);且甲组插管用时短,每次口腔护理时间短,首次插管成功率高,脱管率低,痰痂形成率低,与乙组比较差异均有统计学意义(P<0.01或P<0.05)。结论纤维支气管镜引导下经鼻气管插管安全,成功率高,合并症较少。

목적:관찰경비기관삽관여경구기관삽관대엄중쇠갈환자적림상효과。방법100례Ⅱ형호흡쇠갈수요기관삽관적환자,수궤분위량조,매조50례,갑조채용섬유지기관경인도하경비기관삽관방식건립인공기도,을조채용후경인도하경구기관삽관방식,대비량조혈기분석지표、생명체정변화급삽관정황。결과갑조생명체정변화정황우우을조,차이유통계학의의(P<0.05);차갑조삽관용시단,매차구강호리시간단,수차삽관성공솔고,탈관솔저,담가형성솔저,여을조비교차이균유통계학의의(P<0.01혹P<0.05)。결론섬유지기관경인도하경비기관삽관안전,성공솔고,합병증교소。
Objective To observe the clinical effects of nasotracheal intubation and orotracheal intubation for severe failure patients.Methods A total of 100 patients with type Ⅱ respiratory failure, who needed tracheal intubation, were randomly divided into two group, and each group contained 50 cases. Group A received artificial airway construction by fiberoptic bronchoscope guided nasotracheal intubation, and group B by laryngoscope guided orotracheal intubation. The blood gas analysis index, vital signs, and intubation condition were compared between the two groups.Results Group A had better change in vital signs than group B, and the difference had statistical significance (P<0.05). Moreover, group A had shorter intubation time and oral nursing time, higher success rate of the first intubation time, and lower extubation rate and incidence of sputum scab than group B, and the differences had statistical significance (P<0.01 orP<0.05).Conclusion The application of fiberoptic bronchoscope guided nasotracheal intubation has the advantages of success, high safety, and less complications.
