目的：探讨及分析股骨近端防旋髓内钉治疗老年股骨转子间骨折的运用效果。方法：本院2009年4月-2013年5月住院的48例老年股骨转子间骨折患者应用近端防旋髓内钉的方法进行治疗，分析记录手术时间以及出血量，术后随访6~8个月，观察记录全部患者有无内固定物松动、手术切口感染、深静脉血栓形成、股骨头坏死等情况出现。通过Harris疗效评价标准对所有患者术后的疗效进行评价。结果：全部患者的手术均进行顺利，术后影像学资料证实骨折复位情况良好，手术时间平均（55.7±7.6）min，手术出血量平均为（53.8±9.3）mL，随访期间未出现内固定松动、手术切口感染、深静脉血栓形成、股骨头坏死等情况出现，所有患者骨折均完全愈合。Harris评分优良率为93.75%。结论：股骨近端防旋髓内钉治疗老年股骨转子间骨折疗效确切、手术创伤小、固定牢固、易操作、并发症较少，值得临床进一步推广。
목적：탐토급분석고골근단방선수내정치료노년고골전자간골절적운용효과。방법：본원2009년4월-2013년5월주원적48례노년고골전자간골절환자응용근단방선수내정적방법진행치료，분석기록수술시간이급출혈량，술후수방6~8개월，관찰기록전부환자유무내고정물송동、수술절구감염、심정맥혈전형성、고골두배사등정황출현。통과Harris료효평개표준대소유환자술후적료효진행평개。결과：전부환자적수술균진행순리，술후영상학자료증실골절복위정황량호，수술시간평균（55.7±7.6）min，수술출혈량평균위（53.8±9.3）mL，수방기간미출현내고정송동、수술절구감염、심정맥혈전형성、고골두배사등정황출현，소유환자골절균완전유합。Harris평분우량솔위93.75%。결론：고골근단방선수내정치료노년고골전자간골절료효학절、수술창상소、고정뢰고、역조작、병발증교소，치득림상진일보추엄。
Objective:To investigate and analyze the effects of proximal femoral nail anti-rotation treatment of intertrochanteric fractures.Method:48 cases of hospitalized elderly patients with intertrochanteric fractures were treated with proximal nail anti-rotation methods,operative time and blood loss were recorded.All patients were followed up for 6 to 8 months.The presence or absence of implant loosening,surgical wound infection,deep vein thrombosis,femoral head necrosis circumstances arise were observed and recorded. Efficacy of all patients were evaluated by Harris evaluation criteria.Result:All patients underwent successful surgery and postoperative imaging data confirmed fractures in good condition,the mean operative time was(55.7±7.6)min,mean operative blood loss was(53.8±9.3)mL.Loosening, surgical wound infection,deep vein thrombosis,the situation appears femoral head necrosis,during follow-up didn’t appear,and all patients were completely healed fractures.Harris rated excellent rate was 93.75%.Conclusion:The application of the proximal femoral nail anti-rotation treatment of intertrochanteric fracture has exact effect,surgical trauma,fixation,easy to operate,fewer complications and other advantages,worthy of further promotion.
