目的：探讨初次手法复位治疗后症状加重的良性阵发性位置性眩晕的原因与治疗。方法：2012年1月-2014年11月收治初次复位后24 h自觉眩晕症状加重患者20例，再次行变位眼震试验，并根据诱发试验结果给予Epley管石复位法联合 Barbecue 翻滚复位法治疗。结果：二次变位眼震试验阴性的5例患者1周后有3例完全治愈，2例治疗有效；二次变位眼震试验均为阳性的15例患者1周后有11例完全治愈，4例治疗有效，治愈率73.3%。结论：Epley管石复位法联合Barbecue翻滚复位法治疗后半规管和水平半规管混合性BPPV疗效好。

목적：탐토초차수법복위치료후증상가중적량성진발성위치성현훈적원인여치료。방법：2012년1월-2014년11월수치초차복위후24 h자각현훈증상가중환자20례，재차행변위안진시험，병근거유발시험결과급여Epley관석복위법연합 Barbecue 번곤복위법치료。결과：이차변위안진시험음성적5례환자1주후유3례완전치유，2례치료유효；이차변위안진시험균위양성적15례환자1주후유11례완전치유，4례치료유효，치유솔73.3%。결론：Epley관석복위법연합Barbecue번곤복위법치료후반규관화수평반규관혼합성BPPV료효호。
Objective:To explore the cause and treatment of benign paroxysmal positional vertigo with aggravated symptoms after initial manual reduction.Methods:20 patients with consciously aggravating vertigo symptom for 24 h after initial manual reduction from January 2012 to November 2014 were given deflection of nystagmus test again and were treated with Epley stone restoration method combined Barbecue roll reduction method according to the induced experimental results.Results:There were 3 cases with completely cured and 2 cases with effective treatment in 5 patients with negative secondary deflection of nystagmus test.There were 11 cases with completely cured and 4 cases with effective treatment in 15 patients with positive secondary deflection of nystagmus test and the cure rate was 73.3%.Conclusion:Epley stone restoration method combined Barbecue roll reduction method in the treatment of the mixed BPPV of posterior semicircular canal and horizontal semicircular canal had good curative effect.
