目的:研究左卡尼汀辅助治疗老年社区获得性肺炎( CAP)的疗效。方法选择我科住院的老年社区获得性肺炎患者63例,随机分为常规组33例,实验组30例,两组均给予抗感染、化痰、解痉、平喘的常规治疗,实验组加用左卡尼汀2 g/d,观察两组治疗期间的白细胞、中性粒细胞、降钙素原及C反应蛋白的变化。结果①常规组与实验组患者在一般资料无统计学差异( P>0.05)。②治疗第4天常规组与实验组白细胞及中性粒细胞计数均明显降低(P<0.05),但实验组降低更为显著(P=0.007,P=0.004)。③实验组治疗第3天、第5天及第7天血清PCT浓度、CRP浓度与前次数据相比均有明显差异( P<0.05)。结论左卡尼汀辅助治疗老年社区获得性肺炎可增强抗炎效果,缩短抗炎疗程。

Objective To study the clinical effect of L-carnitine in the auxiliary treatment of elderly patients with community-acquired pneumonia. Methods 66 elderly patients with community-acquired pneumonia were ran-domly divided into two groups, 33 cases in the normal control group and 30 cases in the experimental group. Both of them were given common treatment, including anti-infection, reducing phlegm, relieving spasm and preventing asth-ma, and 2g of L-carnitine was given to the experimental group every day. The changes of white blood cells, neutro-phile granulocyte, procalcitonin and C-reactive protein were observed. Results ① There was no statistical differ-ence in basic clinical information between the two groups (P>0. 05).② Four days after the treatment, white blood cells and neutrophil count decreased obviously in the two groups, but the decrease was more pronounced in the exper-imental group than in the control group(P<0. 05). ③ In the experimental group, there were significant differences in the concentration of serum PCT and CRP at the 3rd, 5th and 7th days of the treatment (P<0. 05). Conclusion L-carnitine can enhance anti-infection effect and shorten the period of anti-inflammatory treatment in the auxiliary treatment of elderly patients with community-acquired pneumonia.
