目的检测3~8岁阻塞性睡眠暂停低通气综合征(obstructive sleep apnea-bypopnea sgn-drome,OSAHS)儿童血清性激素水平,探讨其性激素变化与下丘脑—垂体—性腺轴(hypothalamic-pituitary-gonad axis,HPGA)启动的关系。方法以76例3~8岁符合 OSAHS 的儿童和40例3~8岁健康儿童为研究对象,检测血清睾酮(T)、雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)及催乳素(PRL)水平,对其 HPGA 启动、体格发育情况进行对比,分析性别、低氧、体重指数(BMI)与性激素水平之间的关系。结果OSAHS 组儿童身高、体重水平明显低于对照组(P <0.05),E2水平明显高于对照组(P <0.01)。OSAHS 组男童 FSH 水平明显低于对照组(P <0.01)。OSAHS 组女童 FSH 水平明显高于 OSAHS 组男童(P <0.01)。结论OSAHS 可能影响儿童身高、体重,OSAHS 可能影响 E2、FSH 的分泌,3~8岁 OS-AHS 儿童体内雌激素的负反馈调节敏感性高,HPGA 尚未启动。

목적검측3~8세조새성수면잠정저통기종합정(obstructive sleep apnea-bypopnea sgn-drome,OSAHS)인동혈청성격소수평,탐토기성격소변화여하구뇌—수체—성선축(hypothalamic-pituitary-gonad axis,HPGA)계동적관계。방법이76례3~8세부합 OSAHS 적인동화40례3~8세건강인동위연구대상,검측혈청고동(T)、자이순(E2)、란포자격소(FSH)、황체생성소(LH)급최유소(PRL)수평,대기 HPGA 계동、체격발육정황진행대비,분석성별、저양、체중지수(BMI)여성격소수평지간적관계。결과OSAHS 조인동신고、체중수평명현저우대조조(P <0.05),E2수평명현고우대조조(P <0.01)。OSAHS 조남동 FSH 수평명현저우대조조(P <0.01)。OSAHS 조녀동 FSH 수평명현고우 OSAHS 조남동(P <0.01)。결론OSAHS 가능영향인동신고、체중,OSAHS 가능영향 E2、FSH 적분비,3~8세 OS-AHS 인동체내자격소적부반궤조절민감성고,HPGA 상미계동。
Objetive By detecting the levels of sex hormone levels of preschool children with and with-out obstructive sleep apnea-hypopnea syndrome(OSAHS)of 3 ~8 years old and comparing the differences ,we aimed at further exploring the impacts of OSAHS on the preschool sex hormone levels and puberty starts.Meth-ods 76 children proved by polysomnography (PSG)examples and 40 healthy children (3 ~8 years old)were chosen.The serum level of Follicle-Stimulating Hormone (FSH),Luteinizing Hormone(LH),estradiol(E2 ),pro-lactin(PRL)and testosterone(T)were measured by chemiluminescent microparticle immunoassay analyze.By comparing the differences of sex hermone level and analyzing the relationship between sex、hypoxia、BMI and sex hermone further exploring the condition of puberty starts and physical growth.Results The height and weight of OSAHS group was significant lower than the control group and the difference was statistically significant (P <0.05).OSAHS patient’s E2 level was higher than the control group’s and showed significant statistical difference (P <0.01).Male OSAHS patient's FSH level was significantly lower than male control’s and showed significant statistical difference (P <0.01).Female OSAHS patient’s FSH level was significantly lower than male OSAHS patient’s and showed significant statistical difference (P <0.01). Conclusion OSAHS might influence chil-dren's weight and height;OSAHS might influence E2、FSH’s level,the negative feedback regulation in children of 3 ~8 years old with OSAHS is sensitive,the HPGA in children of 3 ~8 years old with OSAHS is still not open.
