目的：研究道家认知疗法联合抗抑郁药物治疗对伴抑郁的高血压患者的治疗效果及生活质量的影响。方法选择我院高血压患者进行汉密顿抑郁量表（Hamilton Depression Scale ，HAMD）评分，将60例 HAMD评分阳性者（HAMD≥18分）随机分为治疗组采用常规降压治疗、道家认知疗法及抗抑郁药物盐酸舍曲林片治疗；对照组采用常规降压治疗。8周后比较两组治疗后的降压效果、抑郁水平与生活质量。结果与对照组相比治疗组的抑郁情绪有显著缓解，8周后治疗组 HAMD评分由（30.27±5.09）下降至（16.23±3.41）；治疗组收缩压由（163.6±15.1）降至（126.5±14.1） mmHg ，舒张压由（99.8±12.3）降至（73.5±11.6）mmHg ，与对照组相比有显著性差异；治疗组生活质量改善优于对照组，治疗组生活质量总分由（66.42±5.67）上升到（71.57±7.28）。结论道家认知疗法及抗抑郁药物治疗对伴发抑郁情绪的高血压患者治疗效果更好，不仅能更好的控制血压，且能明显提高生活质量，改善抑郁情绪。
목적：연구도가인지요법연합항억욱약물치료대반억욱적고혈압환자적치료효과급생활질량적영향。방법선택아원고혈압환자진행한밀돈억욱량표（Hamilton Depression Scale ，HAMD）평분，장60례 HAMD평분양성자（HAMD≥18분）수궤분위치료조채용상규강압치료、도가인지요법급항억욱약물염산사곡림편치료；대조조채용상규강압치료。8주후비교량조치료후적강압효과、억욱수평여생활질량。결과여대조조상비치료조적억욱정서유현저완해，8주후치료조 HAMD평분유（30.27±5.09）하강지（16.23±3.41）；치료조수축압유（163.6±15.1）강지（126.5±14.1） mmHg ，서장압유（99.8±12.3）강지（73.5±11.6）mmHg ，여대조조상비유현저성차이；치료조생활질량개선우우대조조，치료조생활질량총분유（66.42±5.67）상승도（71.57±7.28）。결론도가인지요법급항억욱약물치료대반발억욱정서적고혈압환자치료효과경호，불부능경호적공제혈압，차능명현제고생활질량，개선억욱정서。
Objective To investigate the effect of antidepressant and Tao cognition therapy on the blood pressure and quality of life in hypertensive patients with depression .Methods After evaluating patients with essential hypertension by Hamilton De- pression Scale (HAMD) ,60 patients who got scored higher than 18 with HAMD were selected and randomly divided into two groups .One group were accepted antidepressant Sertraline and Tao cognition therapy (n=30 ,including routine treatment) and another as control group (n=30 ,routine treatment alone) .The blood pressure ,quality of life ,and level of depression were ob- served and compared between these 2 groups .Results The depression symptoms were significantly improved in the treatment group .The HAMD score fell from (30.27 ± 5.09) to (16.23 ± 3.41) after 8 weeks treatment .The blood pressure control was more effective in the treatment group than in the control group .In the treatment group the systolic blood pressure (SBP) de- creased from (163.6 ± 15.1) to (126.5 ± 14.1) mmHg and the diastolic blood pressure(DBP) decreased from (99.8 ± 12.3) to (73.5 ± 11.6) mmHg ,which were better than those in the control group .And in addition patients in the treatment group got a higher score in the quality of life .The total score of GQOLI-74 increased from (66.42 ± 5.67) to (71.57 ± 7.28) after the Ser- traline and Tao cognition therapy treatment .Conclusion Tao cognition therapy and antidepressant are more effective in hyper- tensive patients with depression ,which can not only improve the blood pressure control and quality of life but also decrease de- pression .
