目的:探讨新疆地区汉族人群转录因子7类似物2( TCF7L2)基因 rs11196218、rs10749127、rs290487、rs290481多态性与2型糖尿病(T2DM)的相关性。方法选取2013年11月—2014年2月新疆医科大学第一附属医院收治的汉族 T2DM 患者164例为 T2DM 组,另选取同时期体检的健康人群195例为对照组,测量并记录身高、体质量、腹围、血压,检测血糖、糖化血红蛋白(HbA1c )、胰岛素、C 肽、血脂、尿酸水平。提取外周血 DNA,检测 TCF7L2基因 rs11196218、rs10749127、rs290487、rs290481基因型及等位基因频率。多因素 Logistic 回归分析T2DM 的影响因素。结果对照组与 T2DM 组收缩压(SBP)比较,差异无统计学意义(P ﹥0.05);T2DM 组患者舒张压(DBP)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL - C)、空腹血糖及 HbA1c 较对照组升高,高密度脂蛋白胆固醇(HDL - C)较对照组降低(P ﹤0.05)。对照组与 T2DM 组 rs11196218、rs290481基因型和等位基因频率分布,差异均无统计学意义( P ﹥0.05);两组 rs10749127 TT 基因型分布,差异有统计学意义( P ﹤0.05), rs290487 CC 基因型分布及等位基因频率分布,差异有统计学意义( P ﹤0.05)。rs10749127的 CC、CT、TT 基因型T2DM 患者间空腹血糖及尿酸水平比较,差异有统计学意义( P ﹤0.05)。rs290487的 CC、CT、TT 基因型间各指标比较,差异均无统计学意义( P ﹥0.05)。多因素 Logistic 回归分析结果显示:rs290487CC 基因型、腰围、体质指数(BMI)、空腹血糖及 HbA1c与 T2DM 有回归关系(P ﹤0.05)。结论 TCF7L2基因 rs10749127、rs290487与新疆地区汉族人群的 T2DM 的发病率有关,未发现 rs11196218、rs290481与 T2DM 发病的关联。

목적:탐토신강지구한족인군전록인자7유사물2( TCF7L2)기인 rs11196218、rs10749127、rs290487、rs290481다태성여2형당뇨병(T2DM)적상관성。방법선취2013년11월—2014년2월신강의과대학제일부속의원수치적한족 T2DM 환자164례위 T2DM 조,령선취동시기체검적건강인군195례위대조조,측량병기록신고、체질량、복위、혈압,검측혈당、당화혈홍단백(HbA1c )、이도소、C 태、혈지、뇨산수평。제취외주혈 DNA,검측 TCF7L2기인 rs11196218、rs10749127、rs290487、rs290481기인형급등위기인빈솔。다인소 Logistic 회귀분석T2DM 적영향인소。결과대조조여 T2DM 조수축압(SBP)비교,차이무통계학의의(P ﹥0.05);T2DM 조환자서장압(DBP)、삼선감유(TG)、총담고순(TC)、저밀도지단백담고순(LDL - C)、공복혈당급 HbA1c 교대조조승고,고밀도지단백담고순(HDL - C)교대조조강저(P ﹤0.05)。대조조여 T2DM 조 rs11196218、rs290481기인형화등위기인빈솔분포,차이균무통계학의의( P ﹥0.05);량조 rs10749127 TT 기인형분포,차이유통계학의의( P ﹤0.05), rs290487 CC 기인형분포급등위기인빈솔분포,차이유통계학의의( P ﹤0.05)。rs10749127적 CC、CT、TT 기인형T2DM 환자간공복혈당급뇨산수평비교,차이유통계학의의( P ﹤0.05)。rs290487적 CC、CT、TT 기인형간각지표비교,차이균무통계학의의( P ﹥0.05)。다인소 Logistic 회귀분석결과현시:rs290487CC 기인형、요위、체질지수(BMI)、공복혈당급 HbA1c여 T2DM 유회귀관계(P ﹤0.05)。결론 TCF7L2기인 rs10749127、rs290487여신강지구한족인군적 T2DM 적발병솔유관,미발현 rs11196218、rs290481여 T2DM 발병적관련。
Objective To investigate the relationship between polymorphisms of transcription factor 7 - like - 2 (TCF7L2) gene ( rs11196218,rs10749127,rs290487,rs290481 ) and type 2 diabetes mellitus ( T2DM ) among Han population in Xinjiang. Methods 164 Han patients with T2DM who were treated in the First Affiliated Hospital of Xinjiang Medical University from November 2013 to February 2014,were included in T2DM group,at the same time,195 healthy people who received physical examination were included in control group. The height,body mass,abdominal circumference,blood pressure,levels of blood glucose,glycated hemoglobin( HbA1c ),insulin,C - peptide,blood lipids and uric acid were measured and recorded. Peripheral blood DNA was extracted,genotype and allele frequency were detected for TCF7L2 gene rs11196218,rs10749127, rs290487, rs290481. The influence factors for T2DM were analysed by multivariable Logistic regression. Results There was no significant difference in systolic blood pressure(SBP)between control group and T2DM group (P ﹥ 0. 05). The diastolic blood pressure(DBP),levels of three acyl glycerin(TG),total cholesterol(TC),low - density lipoprotein cholesterol(LDL - C),fasting blood glucose and HbA1c of patients in T2DM group were significantly higher than those of patients in control group,but the high density lipoprotein cholesterol(HDL - C)level of patients in T2DM group was significantly lower than that of patients in control group(P ﹤ 0. 05). There was no significant difference in genotype and allele frequency distribution of rs11196218 and rs290481 between the control group and T2DM group( P ﹥ 0. 05 ) . There was significant difference in distribution of rs10749127 TT genotype between control group and T2DM group(P ﹤ 0. 05). There were significant differences in genotype and allele frequency distribution of rs290487 CC between control group and T2DM group(P ﹤0. 05). There were significant differences in levels of fasting glucose and uric acid among T2DM patients with different rs10749127 genotypes(CC,CT and TT)(P ﹤ 0. 05). But there was no significant difference in various indexes among CC, CT and TT genotype of rs290487( P ﹥ 0. 05). According to multivariable Logistic regression analysis results,rs290487 CC genotype,abdominal circumference,BMI,levels of fasting blood glucose and HbA1c had correlation with T2DM(P ﹤ 0. 05). Conclusion TCF7L2 gene rs10749127 and rs290487 are correlated with the onset of T2DM among Han population in Xinjiang, but the correlation between rs11196218,rs290481 and T2DM are not found.
