目的：研究微创穿刺术治疗慢性硬膜下血肿的临床疗效。方法回顾我院2011年1月－2014年7月120例慢性硬膜下血肿的患者，随机分成两组，60例采用微创穿刺引流术进行治疗（ A组），60例采用钻孔引流术进行治疗（B组）。结果 A组手术平均时间明显短于B组（P＜0.05）；A组并发症发生率（10．0％）显著低于 B 组（36.7％）（P＜0.05）；A组神经功能恢复优良率（93．3％）明显高于B组（81．7％）（P＜0.05）；结论微创穿刺术治疗慢性硬膜下血肿，能够明显缩短手术平均时间，提高神经功能恢复优良率，降低手术后并发症发生率，是目前具有广阔前景的新技术。

목적：연구미창천자술치료만성경막하혈종적림상료효。방법회고아원2011년1월－2014년7월120례만성경막하혈종적환자，수궤분성량조，60례채용미창천자인류술진행치료（ A조），60례채용찬공인류술진행치료（B조）。결과 A조수술평균시간명현단우B조（P＜0.05）；A조병발증발생솔（10．0％）현저저우 B 조（36.7％）（P＜0.05）；A조신경공능회복우량솔（93．3％）명현고우B조（81．7％）（P＜0.05）；결론미창천자술치료만성경막하혈종，능구명현축단수술평균시간，제고신경공능회복우량솔，강저수술후병발증발생솔，시목전구유엄활전경적신기술。
Objective To study the clinical effect of minimally invasive puncture and drainage in the treatment of chronic subdural hematoma.Methods 120 cases of chronic subdural hematoma patients in our hospital from January,2011 to July,2014 were randomly divided into two groups. 60 cases were treated with minimally invasive puncture drainage treatment (group A),while the other 60 cases with trepanation and drainage for treatment ( group B) .Results In group A, the average operation time was (22.2±7.5) min, while the average operation time in group B was (51.4±10.4) min.The complication rate in group A was 10%,significantly lower than that of group B ( 36.7%) .In group A, the nerve function recovery rate was 93.3%, which was sig-nificantly higher than that in group B (81.7%).There is significant difference between the two groups in the operation time, the average recovery of neurological function and the incidence of postoperative complications in three areas, with statistical significance ( P<0.05 ) .Conclusion The minimally invasive treatment of chronic subdural hematoma can significantly shorten the average time of operation,improve the nerve function recovery,reduce the incidence of compli-cations after operation.As a new technique,it has broad prospects for further application.
