目的:探讨盐酸曲美他嗪辅治老年不稳定型心绞痛( UA)患者后血浆同型半胱氨酸( Hcy)、基质金属蛋白酶-9(MMP-9)和内皮功能的变化。方法将UA患者(年龄≥60岁)150例按随机数字表法分为对照组(75例)和观察组(75例),2组均接受硝酸酯类、阿司匹林、他汀类、β受体阻滞剂等常规,观察组加用盐酸曲美他嗪(20 mg,3次/d),2组治疗1个月,治疗前后检测患者血浆Hcy、MMP-9水平及内皮细胞功能。结果治疗前2组患者Hcy、MMP-9与内皮功能无显著差异( P﹥0?.05);治疗1个月后,2组Hcy、MMP-9、内皮素-1( ET-1)和血管性血友病因子(vWF)均明显降低( P <0.05),一氧化氮(NO)及肱动脉血流介导的内皮依赖性舒张变化率(FMD)均明显升高(P <0.05)。且观察组改善均优于对照组( P <0.05)。结论盐酸曲美他嗪通过降低Hcy、ET-1与vWF水平、增加NO水平改善血管内皮功能,同时降低MMP-9水平预防心肌重塑。

목적:탐토염산곡미타진보치노년불은정형심교통( UA)환자후혈장동형반광안산( Hcy)、기질금속단백매-9(MMP-9)화내피공능적변화。방법장UA환자(년령≥60세)150례안수궤수자표법분위대조조(75례)화관찰조(75례),2조균접수초산지류、아사필림、타정류、β수체조체제등상규,관찰조가용염산곡미타진(20 mg,3차/d),2조치료1개월,치료전후검측환자혈장Hcy、MMP-9수평급내피세포공능。결과치료전2조환자Hcy、MMP-9여내피공능무현저차이( P﹥0?.05);치료1개월후,2조Hcy、MMP-9、내피소-1( ET-1)화혈관성혈우병인자(vWF)균명현강저( P <0.05),일양화담(NO)급굉동맥혈류개도적내피의뢰성서장변화솔(FMD)균명현승고(P <0.05)。차관찰조개선균우우대조조( P <0.05)。결론염산곡미타진통과강저Hcy、ET-1여vWF수평、증가NO수평개선혈관내피공능,동시강저MMP-9수평예방심기중소。
Obj ective To investigate the levels of plasma homocysteine ( Hcy) levels, matrix metalloproteinase-9 (MMP-9) and the endothelial function after use the trimetazidine hydrochloride in treatment of elderly patients with unstable angina pectoris ( UA) .Methods Totally 150 cases with UA patients ( age≥60 years ) were randomly divided into control group (75 cases) and observation group (75 cases)by random number table,2 groups received nitrates, aspirin, statins, beta blockers and other conventional , the observation group added trimetazidine hydrochloride (20 mg, 3 times/d), 2 groups re-ceived 1 month of treatment , level of plasma Hcy , MMP-9 and endothelial cells function before and after treatment were test-ed.Results There were no significant difference of Hcy , MMP-9 and endothelial function between the 2 groups before treat-ment ( P >0.05);after 1 month treatment, 2 groups'Hcy, MMP-9, endothelin-1 (ET-1) and von Willebrand factor (vWF) were significantly decreased ( P <0.05), nitric oxide (NO) and brachial artery flow mediated endothelial dependent dilata-tion rate (FMD) were significantly increased ( P <0.05).The observation group was better than the control group ( P <0.05).Conclusion Trimetazidine hydrochloride can improve endothelial function by reducing Hcy , ET1-and vWF levels, and increasing NO levels , and reduced MMP-9 level to prevent of myocardial remodeling .
