目的：分析丙型肝炎病毒（HCV）基因型与慢性丙型肝炎患者病情严重程度之间关系，为慢性丙型肝炎患者的临床诊断和治疗提供依据。方法采用直接测序法对 HCV进行基因分型，并测量患者未治疗前 HCV‐RNA水平、丙氨酸氨基转移酶（ALT）、天门冬氨酸氨基转移酶（AST）、清蛋白（ALB）、血红蛋白（Hb）、血小板计数（PLT ）水平。结果共对321株HCV进行基因分型，其中1a型占1．2％，1b型占78．5％，2a型占8．7％，3a型占2．2％，3b型占4．7％，6a型占2．8％，6b型占1．9％。 HCV1型患者ALT、HCV‐RNA、AST／PLT比值指数（APRI）水平明显高于非1型患者，差异有统计学意义（P值分别为0．009，＜0．01，0．036）；而AST、ALB、Hb、PLT、AST／ALT水平两者并无明显差异。结论 HCV1型患者 HCV‐RNA水平高于非1型患者，肝脏功能损害较非1型患者严重。
목적：분석병형간염병독（HCV）기인형여만성병형간염환자병정엄중정도지간관계，위만성병형간염환자적림상진단화치료제공의거。방법채용직접측서법대 HCV진행기인분형，병측량환자미치료전 HCV‐RNA수평、병안산안기전이매（ALT）、천문동안산안기전이매（AST）、청단백（ALB）、혈홍단백（Hb）、혈소판계수（PLT ）수평。결과공대321주HCV진행기인분형，기중1a형점1．2％，1b형점78．5％，2a형점8．7％，3a형점2．2％，3b형점4．7％，6a형점2．8％，6b형점1．9％。 HCV1형환자ALT、HCV‐RNA、AST／PLT비치지수（APRI）수평명현고우비1형환자，차이유통계학의의（P치분별위0．009，＜0．01，0．036）；이AST、ALB、Hb、PLT、AST／ALT수평량자병무명현차이。결론 HCV1형환자 HCV‐RNA수평고우비1형환자，간장공능손해교비1형환자엄중。
Objective To analyze the relationship between hepatitis C virus (HCV) genotypes and the severity degree of the patients with hepatitis C in order to provide the basis for the diagnosis and treatment of chronic hepati‐tis C(CHC) .Methods The genotype of HCV was tested by the direct sequencing method ,and the levels of HCV‐RNA ,ALT ,AST ,ALB ,Hb ,PLT ,AST/ALT and AST to ptatelet ratio index (APRI) before treatment were deter‐mined .Results 321 strains of HCV were performed the genotyping ,in which ,1b ,2a ,3b ,6a ,3a ,6b and 1a accounted for 78 .5% ,8 .7% ,4 .7% ,2 .8% ,2 .2% ,1 .9% and 1 .2% respectively .The levels of ALT ,HCV‐RNA ,APRI in the patients with HCV genotype 1 were higher than those in the patients with non‐genotype 1 ,the differences had statis‐tical significance(P<0 .01);while the levels of AST ,ALB ,Hb ,PLT and AST/ALT had no statistical differences be‐tween the two groups .Conclusion The level of HCV‐RNA in the patients with genotype 1 is higher than that with non‐genotype1 ,and the liver damage is severer.
