目的：探讨游离股前外侧穿支皮瓣修复四肢皮肤软组织缺损创面的方法和临床效果。方法：2010年6月~2013年12月收治创伤后肢体皮肤软组织缺损患者19例，其中男15例，女4例；年龄13~55岁，平均33岁；伤后至入院时间为1 h~55 d；损伤部位：小腿9例，足部5例，前臂2例，手部3例；缺损范围3 cm ×8 cm~13 cm ×25 cm，均采用游离股前外侧穿支皮瓣修复创面。记录手术时间、手术出血量、输血量、术后并发症发生率、皮瓣稳定时间、二次手术整形率、患者满意度，随访6~24个月。结果：纳入病例19例，平均手术时间（239±35）min；平均术中出血量（236±45）mL，平均输浓缩红细胞（0.74±0.3）U；术后并发症发生率9%，皮瓣稳定时间（6.4±1.2）d；术后需二次手术整形修整皮瓣1例，修整率5.1%。术后8个月11例获保护性感觉，患者满意率95%。结论：游离股前外侧穿支皮瓣是修复四肢皮肤软组织缺损的理想方法。
목적：탐토유리고전외측천지피판수복사지피부연조직결손창면적방법화림상효과。방법：2010년6월~2013년12월수치창상후지체피부연조직결손환자19례，기중남15례，녀4례；년령13~55세，평균33세；상후지입원시간위1 h~55 d；손상부위：소퇴9례，족부5례，전비2례，수부3례；결손범위3 cm ×8 cm~13 cm ×25 cm，균채용유리고전외측천지피판수복창면。기록수술시간、수술출혈량、수혈량、술후병발증발생솔、피판은정시간、이차수술정형솔、환자만의도，수방6~24개월。결과：납입병례19례，평균수술시간（239±35）min；평균술중출혈량（236±45）mL，평균수농축홍세포（0.74±0.3）U；술후병발증발생솔9%，피판은정시간（6.4±1.2）d；술후수이차수술정형수정피판1례，수정솔5.1%。술후8개월11례획보호성감각，환자만의솔95%。결론：유리고전외측천지피판시수복사지피부연조직결손적이상방법。
Objective:To explore the therapeutic effect of anterolateral thigh perforator free flaps for reconstruction of four limb soft tissue defects. Methods:Among 19 patients were treated with anteriolateral thigh perforator free flap for traumat-ic soft tissue defects from June 2010 to December 2013,15 cases were male,female 4 cases;the age of the 19 patients was ranging from 13 to 55 years old,and the average age was 33. The time between the injury and the operation was 1 h~55 d. The damaged part:9 cases in leg,5 cases in foot,2cases in forearm and 3 cases in hand. The sizes of those soft tissue defects were from 3 cm × 8 cm to 13 cm × 35 cm. Operation time,blood loss,blood transfusion,postoperative complications,flap stabilization time,second plastic surgery rates and patient satisfaction were recorded. All the patient were followed up for 6 to 24 months. Results:All flaps survived completely,the mean operation time were( 239 ± 35 ) mins,the mean blood loss were(236 ± 45)mL,the mean blood transfusion were(0. 74 ± 0. 3)U,the postoperative complication rate was 9%,the mean time for flap stabilization were(6. 4 ± 1. 2)d,the satisfaction rate of patients was 95%,reoperation rate was 5. 1%. Conclusion:The anteriolateral thigh perforator free flaps is an ideal method for repai-ring the soft tissue defects in extremities.
