目的:探讨血清降钙素原(PCT)检测在慢性阻塞性肺疾病急性加重期(AECOPD)治疗中的应用价值。方法将90例 AECOPD 患者随机分为两组,各45例。观察组根据 PCT 检测指标给予抗生素治疗,对照组根据超敏 C 反应蛋白(hs-CRP)和(或)白细胞(WBC)指标给予抗生素治疗,观察两组治疗前及治疗8 d 后 PCT、hs-CRP 及 WBC 指标变化,以及抗生素使用时间、住院时间及治疗效果等。结果治疗8 d 后,两组 PCT、hs-CRP 及 WBC 指标均较治疗前明显下降( P <0.01),且观察组 PCT 水平低于对照组( P <0.01);观察组抗生素使用时间及住院时间均短于对照组( P <0.05或 P <0.01)。结论PCT 检测对 AECOPD 抗生素的使用具有较高的临床指导价值,可规范抗生素的合理应用,缩短抗生素的使用时间以及患者的住院时间。

목적:탐토혈청강개소원(PCT)검측재만성조새성폐질병급성가중기(AECOPD)치료중적응용개치。방법장90례 AECOPD 환자수궤분위량조,각45례。관찰조근거 PCT 검측지표급여항생소치료,대조조근거초민 C 반응단백(hs-CRP)화(혹)백세포(WBC)지표급여항생소치료,관찰량조치료전급치료8 d 후 PCT、hs-CRP 급 WBC 지표변화,이급항생소사용시간、주원시간급치료효과등。결과치료8 d 후,량조 PCT、hs-CRP 급 WBC 지표균교치료전명현하강( P <0.01),차관찰조 PCT 수평저우대조조( P <0.01);관찰조항생소사용시간급주원시간균단우대조조( P <0.05혹 P <0.01)。결론PCT 검측대 AECOPD 항생소적사용구유교고적림상지도개치,가규범항생소적합리응용,축단항생소적사용시간이급환자적주원시간。
Objective To investigate the clinical value of the serum procalcitonin (PCT) in chronic obstructive pulmonary disease with acute exacerbation (AECOPD).Methods Nitety AECOPD patients were randomly divided into two groups with 45 cases in each group.The observation group was given antibiotic treatment according to the levels of PCT .The control group was given guided treatment on the basis of high sensitivity C reactive protein (hs-CRP) and (or) white blood cell (WBC) level.For both groups,PCT, hs-CRP and WBC and detailed records of time of the use of antibiotics ,hospitalization time and treatment efficacy before treatment and eight day after treatment were observed.Results On 8 d after treatment,the levels of PCT, hs-CRP,WBC,and hospitalization time were much lower than that before treatment ( P <0.01).PCT level of the observation group was lower than that of the control group (P <0.01),but the time of use of antibiotics and hospitalization time were shorter than that of control group ( P <0.05 or P <0.01).Conclusion PCT Monitoring has higher clinical value for the use of AECOPD antibiotics and can guide the antibiotic applica -tion,standardize the reasonable application of antibiotics , reduce the time of use of antibiotics and hospitalization time .
