目的：探讨利福喷丁联合标准抗结核药物治疗老年肺结核的临床效果及安全性。方法将70例初治老年肺结核患者随机分为两组，各35例。对照组给予标准抗结核药物治疗，观察组将标准抗结核药物中利福平更换为利福喷丁。治疗3个月后，对比两组临床治疗效果。结果观察组治疗总有效率为94．3％，高于对照组的77．1％（ P ＜0．05）。两组治疗前天门冬氨酸氨基转移酶（AST）和丙氨酸氨基转移酶（ALT）水平差异均无统计学意义（ P ＞0．05）；治疗后两组 AST 和 ALT 水平均较治疗前升高，但观察组 AST 和 ALT 水平低于对照组（ P ＜0．01）。两组不良反应发生率比较，观察组胃肠道反应及白细胞降低发生率均低于对照组（ P ＜0．05）。结论利福喷丁联合标准抗结核药物治疗老年肺结核临床效果确切，且安全可靠，值得临床推广应用。
목적：탐토리복분정연합표준항결핵약물치료노년폐결핵적림상효과급안전성。방법장70례초치노년폐결핵환자수궤분위량조，각35례。대조조급여표준항결핵약물치료，관찰조장표준항결핵약물중리복평경환위리복분정。치료3개월후，대비량조림상치료효과。결과관찰조치료총유효솔위94．3％，고우대조조적77．1％（ P ＜0．05）。량조치료전천문동안산안기전이매（AST）화병안산안기전이매（ALT）수평차이균무통계학의의（ P ＞0．05）；치료후량조 AST 화 ALT 수평균교치료전승고，단관찰조 AST 화 ALT 수평저우대조조（ P ＜0．01）。량조불량반응발생솔비교，관찰조위장도반응급백세포강저발생솔균저우대조조（ P ＜0．05）。결론리복분정연합표준항결핵약물치료노년폐결핵림상효과학절，차안전가고，치득림상추엄응용。
Objective To investigate the clinical efficacy and safety of rifapentine and standard anti -TB drugs of combination therapy in treatment of senile tuberculosis .Methods Seventy elderly patients with tuberculosis were randomly divided into two groups (n =35,each group).The control group received standard anti -TB drugs,and the observation group changed standard anti -TB drugs with rifapentine medication.The clinical efficacy was evaluated 3 months after the treatment.Results The total efficacy rate of the ob-servation group was 94.3% higher than 77.1% of the control group ( P <0.05).Before treatment,the AST and ALT levels of both the groups showed no significant difference ( P >0.05), but after treatment,the AST and ALT levels of both the groups increased with that of observation group lower than that of the control group ( P <0.01).The adverse reaction rate and gastrointestinal reaction rate and and leucopenia rate of the observation group were lower than those of the control group ( P <0.05).Conclusion The rifapentine and standard anti-TB drugs of combination therapy in treatment of senile tuberculosis is safe ,effective and worthy of clinical application .
