目的：探讨手术显微镜直视下视网膜裂孔定位、冷凝在外路孔源性视网膜脱离手术中的应用及治疗效果。方法35例（35只眼）孔源性视网膜脱离行外路手术，术中均采用巩膜扣带、外放液、手术显微镜直视下视网膜裂孔定位、冷凝。结果33例眼硅胶垫压准确，视网膜复位良好，无异常炎症反应。34例肉眼下无黄斑部视网膜下积液，黄斑部OCT随访3～6个月。结论手术显微镜直视下裂孔定位准确，操作简单，疗效确切。

목적：탐토수술현미경직시하시망막렬공정위、냉응재외로공원성시망막탈리수술중적응용급치료효과。방법35례（35지안）공원성시망막탈리행외로수술，술중균채용공막구대、외방액、수술현미경직시하시망막렬공정위、냉응。결과33례안규효점압준학，시망막복위량호，무이상염증반응。34례육안하무황반부시망막하적액，황반부OCT수방3～6개월。결론수술현미경직시하렬공정위준학，조작간단，료효학절。
Objective To discuss the application and therapeutic? efficacy? of retinal breaks localization and cryocoagulation under direct surgical microscope visualization in external route rhegmatogenous retinal detachment surger -ies.Methods Totally 35 patients received external route surgery for rhegmatogenous retinal detachment .Standard proce-dures included scleral buckling , subretinal fluid release , localization of retinal breaks under direct vision with surgical mi-croscope , and cryocoagulation .Results All 35 cases had accurate scleral buckling pad position and favorable retinal reat-tachment without inflammatory reactions .Optical coherence? tomography? ( OCT ) showed that up to 6 months after the surgery, there was no macular sub-retinal edema in 34 cases.Conclusion Localization of retinal breaks under direct sur-gical microscope visualization provides accurate positioning and reliable outcomes .
