目的:分析缺氧诱导因子( HIF)在妊娠高血压疾病患者胎盘滋养细胞中的表达差异,评价其临床价值。方法选取重庆市沙坪坝区人民医院2013年1月至2013年12月分娩孕妇170例,分为正常对照组50例,妊娠高血压疾病组85例,轻度子痫前期组22例,重度子痫前期组13例。应用免疫组织化学方法及Western blot法,比较各组胎盘组织HIF-1α的表达和HIF-1α/β-actin的相对吸光度比值;免疫组织化学方法比较各组胎盘组织中HIF-2蛋白的表达,采用RT-PCR方法检测HIF-2αmRNA水平。结果妊娠高血压疾病组、轻度子痫前期组和重度子痫前期组的HIF-1α和HIF-1α/β-actin的相对吸光度比值均显著高于正常组( t值分别为11.776、17.216、21.168、9.979、13.171、21.406,均P<0.01)。重度子痫前期组的HIF-1α和HIF-1α/β-actin的相对吸光度比值显著高于轻度子痫前期组和妊娠高血压疾病组(t值分别为7.372、7.449、11.623、23.338,均P<0.01)。妊娠高血压疾病组、轻度子痫前期组和重度子痫前期组的HIF-2αmRNA及蛋白均显著高于正常组( t值分别为17.344、29.253、34.833、39.388、41.850、38.567,均P<0.01);重度子痫组的HIF-2αmRNA及蛋白显著高于轻度子痫前期组和妊娠高血压疾病组( t值分别为6.557、6.348、30.263、19.314,均P<0.01)。 HIF-1α在妊娠高血压疾病组、轻度子痫前期组和重度子痫前期组中的表达率均显著高于正常组(χ2值分别为8.434、9.680、4.225,均P<0.05),HIF-1α在轻度子痫前期组的表达率显著高于妊娠高血压疾病组(χ2=4.919,P<0.05),HIF-1α在轻度子痫前期组的表达率和重度子痫前期组无显著差异(χ2=0.000,P>0.05)。结论缺氧诱导因子HIF-1α和HIF-2α在妊娠期高血压疾病中具有重要意义。

Objective To analyze the expression changes and clinical value of hypoxia-inducible factor in placental trophoblast cells with pregnancy-induced hypertension .Methods From January 2013 to December 2013 170 cases of pregnant women giving birth in Chongqing Municipal People ’ s Hospital of Shapingba District were divided into normal control group ( 50 cases ) , pregnancy-induced hypertension group (85 cases), mild preeclampsia group (22 cases) and severe preeclampsia group (13 cases).Immunohistochemical methods and Western blot method were used to compare the expression of HIF-1αand HIF-1α/β-actin relative absorbance ratio in each group .The expression of HIF-2 protein was compared among different groups by immunohistochemical methods , and HIF-2αmRNA level was detected by RT-PCR method.Results The expression of HIF-1αand relative absorbance ratio of HIF-1α/β-actin of pregnancy-induced hypertension group, mild preeclampsia group and severe preeclampsia group were significantly higher than those of the normal group ( t value was 11.776, 17.216, 21.168, 9.979, 13.171 and 21.406, respectively, all P<0.01).In severe preeclampsia group the expression of HIF-1αand HIF-1α/β-actin relative absorbance ratio were significantly higher than those in mild preeclampsia group and pregnancy -induced hypertension group (t value was 7.372, 7.449, 11.623 and 23.338, respectively, all P<0.01).In pregnancy-induced hypertension group, mild preeclampsia group and severe preeclampsia group HIF-2αmRNA and protein were significantly higher than those in the normal group (t value was 17.344, 29.253, 34.833, 39.388, 41.850 and 38.567, respectively, all P<0.01).HIF-2αmRNA and protein in severe preeclampsia group were significantly higher than those in mild preeclampsia group and pregnancy -induced hypertension group (t value was 6.557, 6.348, 30.263 and 19.314, respectively, all P <0.01).HIF-1αexpression in pregnancy-induced hypertension group , mild preeclampsia group and severe preeclampsia group was significantly higher than that in normal group (χ2 value was 8.434, 9.680 and 4.225, respectively, all P<0.05), and it was higher in mild preeclampsia group than in pregnancy-induced hypertension group (χ2 =4.919,P<0.05).There was no significant difference in HIF-1αexpression between mild preeclampsia group and severe preeclampsia group (χ2 =0.000, P>0.05).Conclusion The hypoxia-inducible factors HIF-1αand HIF-2αhave significance in pregnancy-induced hypertension .
