目的：探究B超在诊断不同分期宫颈癌宫旁浸润和淋巴转移中的意义。方法将212例宫颈癌患者根据术前检查方式不同分为CT组和B超组，每组106例。对2组患者在术前的宫旁浸润和淋巴结转移的检出结果与术后病理结果进行比较。结果 CT组检查患者有无宫旁浸润的灵敏度为69．2％，特异性为89．2％，Youden指数为0．585， CT检查患者宫旁浸润有19例呈阳性，而病理检出13例，高估6例；B超组检查患者有无宫旁浸润的灵敏度为63．6％，特异性为95．8％，Youden指数为0．594，病理检查与B超检查结果一致。 CT组检查患者淋巴结转移的灵敏度为69．2％，特异性为95．0％，Youden指数为0．642，CT检出22例，病理检出26例，低估4例，20例重合；B超组检查患者淋巴结转移的灵敏度为39．3％，特异性为96．2％，Youden指数为0．354，B超检出14例，病理检出28例，低估14例。结论 B超在诊断不同分期宫颈癌患者宫旁浸润时检出率高，临床应用价值高。
목적：탐구B초재진단불동분기궁경암궁방침윤화림파전이중적의의。방법장212례궁경암환자근거술전검사방식불동분위CT조화B초조，매조106례。대2조환자재술전적궁방침윤화림파결전이적검출결과여술후병리결과진행비교。결과 CT조검사환자유무궁방침윤적령민도위69．2％，특이성위89．2％，Youden지수위0．585， CT검사환자궁방침윤유19례정양성，이병리검출13례，고고6례；B초조검사환자유무궁방침윤적령민도위63．6％，특이성위95．8％，Youden지수위0．594，병리검사여B초검사결과일치。 CT조검사환자림파결전이적령민도위69．2％，특이성위95．0％，Youden지수위0．642，CT검출22례，병리검출26례，저고4례，20례중합；B초조검사환자림파결전이적령민도위39．3％，특이성위96．2％，Youden지수위0．354，B초검출14례，병리검출28례，저고14례。결론 B초재진단불동분기궁경암환자궁방침윤시검출솔고，림상응용개치고。
Objective To investigate the significance of B ultrasound in the diagnosis of different stages of parametrial infiltration and lymph node metastasis in cervical cancer .Methods 212 cases of cervical cancer patients ,according to the preop-erative examination methods ,were divided into the CT group and the B ultrasound group ,106 cases in each group .The preopera-tive parametrial infiltration and lymph node metastasis detection results and pathological results of the 2 groups were compared . Results In CT Group,sensitivity of parametrial infiltration was 69.2%,specificity was 89.2%,Youden index was 0.585,CT ex-amination showed that 19 cases were positive in parametrial infiltration ,and 13 cases were confirmed by pathological examination , 6 cases were overestimated;in B ultrasound group,sensitivity of parametrial infiltration was 63.6%,specificity was 95.8%, Youden index was 0.594,pathological examination and B ultrasound examination results were consistent ;In CT Group,sensitivity of lymph node metastasis was 69.2%,specificity was 95.0%,Youden index was 0.642,CT detected 22 cases,26 cases by patho-logical examination,4 cases were underestimated,and 20 cases of coincidence,in B ultrasound group,sensitivity of lymph node metastasis was 39.3%,specificity was 96.2%,Youden index was 0.354,B ultrasound detected 14 cases,28 cases by pathological examination,underestimated in 14 cases.Conclusion B ultrasound in the diagnosis of different stages of parametrial infiltration has high detection rate ,it has high clinical value .
