目的：探讨卡培他滨（xeloda）联合奥沙利铂（L-OHP）治疗进展期胃癌老年患者的客观疗效和不良反应。方法以60例进展期老年胃癌患者为研究对象，采用XELOX方案，即静脉滴注奥沙利铂85 mg／m2，d1；卡培他滨1250 mg／m2，分2次口服，d1～14；以28 d为1个化疗周期，连用3个周期后评价该治疗方案的疗效和毒性以及患者的生活质量改善状况。结果60例患者均可评价，其中5例完全缓解，25例部分缓解，14例处于稳定期，16例患者处于进展期，总有效率为50．0％；不良反应为手足综合征、神经毒性、骨髓抑制和胃肠道反应，均为可逆性。结论奥沙利铂联合卡培他滨治疗进展期老年胃癌患者疗效肯定，安全性较好，患者耐受性高，可改善患者的生活质量。
목적：탐토잡배타빈（xeloda）연합오사리박（L-OHP）치료진전기위암노년환자적객관료효화불량반응。방법이60례진전기노년위암환자위연구대상，채용XELOX방안，즉정맥적주오사리박85 mg／m2，d1；잡배타빈1250 mg／m2，분2차구복，d1～14；이28 d위1개화료주기，련용3개주기후평개해치료방안적료효화독성이급환자적생활질량개선상황。결과60례환자균가평개，기중5례완전완해，25례부분완해，14례처우은정기，16례환자처우진전기，총유효솔위50．0％；불량반응위수족종합정、신경독성、골수억제화위장도반응，균위가역성。결론오사리박연합잡배타빈치료진전기노년위암환자료효긍정，안전성교호，환자내수성고，가개선환자적생활질량。
Objective To investigate the efficacy and toxicities of capecitabin ( xeloda ) combined with oxaliplatin ( L-OHP) for advanced gastric cancer in elderly patients .Methods 60 cases of elderly patients with advanced gastric carcinoma were treated with XELOX regimen(oxaliplation 85 mg/m2 in 4-hour infusion on d1,capecitabine 1 250 mg/m2 from d1 to d14). The cycles were repeated every 28 days.All 60 patients were evaluated after having received 3 cycles of chemotherapy .Results The overall response rate was 50.0%(including 5 CR,25 PR,14 SD,and 16 PD).The main toxicities were hand-foot syndrome, neurotoxicity ,bone marrow suppression and gastrointestinal reactions , all of which were reversible .Conclusion Capecitabine combined with oxaliplatin is an effective and safe treatment for advanced gastric cancer in elderly patients ,with tolerable toxici-ties,and it can improve quality of life of patients .
