目的：比较三维适形放疗同步化疗与单纯三维适形放疗对中晚期食管癌患者生存质量的影响，治疗期间并发症及临床疗效。方法选取206例中晚期食管癌患者作为研究对象，将其中106例行单纯三维适形放疗患者列为对照组，100例行三维适形放疗同步化疗患者为观察组，比较2组患者治疗期间及治疗结束后生存质量各维度的改变，并发症发生率及临床疗效。结果①治疗结束时对照组躯体功能、角色功能、总体症状明显好于观察组（ P＜0．05）。但治疗结束后3个月，观察组躯体功能、角色功能、情绪功能、社会功能以及总体症状均显著优于对照组（ P＜0．05）。②2组患者均未发生严重的4级不良反应，放射性食管炎和放射性肺损伤组间差异不明显（ P＞0．05）；观察组患者白细胞降低、血小板减少、肝功能异常、心脏毒性的发生率显著高于对照组（ P＜0．05）。观察组临床疗效明显优于对照组（ P＜0．05）。结论三维适形放疗同步化疗方案治疗期间会影响患者的生存质量评分，但治疗后短期内生存质量即可逐渐恢复，相对于单纯放疗的患者获益更多。
목적：비교삼유괄형방료동보화료여단순삼유괄형방료대중만기식관암환자생존질량적영향，치료기간병발증급림상료효。방법선취206례중만기식관암환자작위연구대상，장기중106례행단순삼유괄형방료환자렬위대조조，100례행삼유괄형방료동보화료환자위관찰조，비교2조환자치료기간급치료결속후생존질량각유도적개변，병발증발생솔급림상료효。결과①치료결속시대조조구체공능、각색공능、총체증상명현호우관찰조（ P＜0．05）。단치료결속후3개월，관찰조구체공능、각색공능、정서공능、사회공능이급총체증상균현저우우대조조（ P＜0．05）。②2조환자균미발생엄중적4급불량반응，방사성식관염화방사성폐손상조간차이불명현（ P＞0．05）；관찰조환자백세포강저、혈소판감소、간공능이상、심장독성적발생솔현저고우대조조（ P＜0．05）。관찰조림상료효명현우우대조조（ P＜0．05）。결론삼유괄형방료동보화료방안치료기간회영향환자적생존질량평분，단치료후단기내생존질량즉가축점회복，상대우단순방료적환자획익경다。
Objective To compare the life quality , clinical efficacy and complications between the three-dimensional conformal radiotherapy (3-DCRT) combined with chemotherapy and 3-DCRT for esophageal cancer .Methods 206 cases of e-sophageal cancer were selected in this study .106 cases of patients treated with 3-DCRT was the control group,and 100 cases of patients treated by 3-DCRT combined with chemotherapy was the observation group .Life quality,clinical efficacy and toxicities between the 2 groups were compared .Results ①At the end of treatment ,somatic function ,role function and general symptoms in the control group were higher than those of the observation group (P<0.05).But 3 months after treatment,somatic function,role function,emotional function,social function and general symptoms in the observation group were higher than those of the control group(P<0.05).②There were no stage 4 toxic reaction in the 2 groups,and the rates of radiation esophagitis ,radiation pneumo-nitis had no obvious difference between the 2 groups.The rate of leukopenia,thrombocytopenia,liver dysfunction and cardiac tox-icity in the observation group were higher than those of the control group .The clinical efficacy in the observation group were better than that of the control group(P<0.05).Conclusion 3-DCRT combined with chemotherapy affects the life quality of patients during the treatment ,but in the short term it will gradually restore the quality of life after treatment and had more benefits .
