目的：探讨胸腔镜下肺大泡切除术对机体应激反应的影响，积累有效经验以指导临床实践。方法：选取2012年6月-2014年5月我院诊治的Ⅰ～Ⅲ级肺大泡患者60例作为本次观察对象；分别纳为观察组（30例）和对照组（30例）；观察组予以胸腔镜肺大泡切除术治疗；对照组采用开胸肺大泡切除术治疗；观察应激反应。结果：检查前、中、后：（1）观察组的 BP 、HR 和 SpO2相对平稳，前后无明显差异（P＞0．05）；对照组 BP 、HR 波动明显（P＜0．05）；组间比较中，术中的 BP 、HR 及术后 BP 差异明显（P＜0．05）。（2）在皮质醇、血糖、肾上腺素、去甲状腺素水平上，两组患者术中、术后的血糖较术前均明显升高（P＜0．05）；术前、中、后对照组变化明显高于观察组（P＜0．05）。结论：在肺大泡切除术中，胸腔镜方式对患者机体的应激反应相对较轻，创伤小、恢复快，有助于提升患者的生活质量，有利于患者的早日康复，具有临床推广价值。
목적：탐토흉강경하폐대포절제술대궤체응격반응적영향，적루유효경험이지도림상실천。방법：선취2012년6월-2014년5월아원진치적Ⅰ～Ⅲ급폐대포환자60례작위본차관찰대상；분별납위관찰조（30례）화대조조（30례）；관찰조여이흉강경폐대포절제술치료；대조조채용개흉폐대포절제술치료；관찰응격반응。결과：검사전、중、후：（1）관찰조적 BP 、HR 화 SpO2상대평은，전후무명현차이（P＞0．05）；대조조 BP 、HR 파동명현（P＜0．05）；조간비교중，술중적 BP 、HR 급술후 BP 차이명현（P＜0．05）。（2）재피질순、혈당、신상선소、거갑상선소수평상，량조환자술중、술후적혈당교술전균명현승고（P＜0．05）；술전、중、후대조조변화명현고우관찰조（P＜0．05）。결론：재폐대포절제술중，흉강경방식대환자궤체적응격반응상대교경，창상소、회복쾌，유조우제승환자적생활질량，유리우환자적조일강복，구유림상추엄개치。
Objective :To investigate the thoracoscopic pulmonary bulla resection effects on stress response ,effective accumulated experience to guide clinical practice .Methods :From June 2012 to May 2014 in our hospital ,60 cases of pa‐tients with Ⅰ ~ Ⅲ pulmonary bullae as the object of observation ;respectively into observation group (30 cases) and control group (30 cases) ;observation group were given thoracoscopic pulmonary bulla resection therapy ;control group by thoracotomy pulmonary bulla resection in the treatment of observation ;stress reaction .Results :(1)The observation group BP ,HR and SpO2 are relatively stable ,and no significant difference (P> 0 .05) ;control group BP ,HR fluctua‐tions significantly (P< 0 .05) ;comparison between groups ,differences in BP ,HR and BP in resection of significantly (P< 0 .05) .(2) Glucose ,adrenaline ,cortisol in to thyroxine level ,patients in the two groups after operation ,blood glucose were increased significantly (P< 0 .05) ;preoperative ,in control group was significantly higher than that after the change ,the observation group (P< 0 .05) .Conclusion :Pulmonary bulla resection ,thoracoscopic methods on stress reaction of patients with body relatively light ,small trauma ,quick recovery ,help to improve the quality of life of pa‐tients ,and help the patients recover at an early date ,has clinical value .
