目的::探讨应用无创正压通气( NIPPV)治疗慢性阻塞性肺病( COPD)急性加重期患者的临床疗效。方法:选择COPD急性加重期患者68例,随机分为对照组和NIPPV组,对照组患者34例,采用常规治疗;NIPPV组患者34例,采用常规联合NIPPV辅助治疗。观察两组患者症状、体征及PaCO2、PaO2、pH值等指标。结果:与治疗前相比,两组患者症状、体征均有好转,PaCO2、PaO2、pH值均有改善(P<0.01);上述指标两组患者组间比较,NIPPV组优于对照组,差异有统计学意义(P<0.05)。结论:应用无创正压通气辅助治疗COPD急性加重期病例的临床疗效优于常规治疗,值得推广。

목적::탐토응용무창정압통기( NIPPV)치료만성조새성폐병( COPD)급성가중기환자적림상료효。방법:선택COPD급성가중기환자68례,수궤분위대조조화NIPPV조,대조조환자34례,채용상규치료;NIPPV조환자34례,채용상규연합NIPPV보조치료。관찰량조환자증상、체정급PaCO2、PaO2、pH치등지표。결과:여치료전상비,량조환자증상、체정균유호전,PaCO2、PaO2、pH치균유개선(P<0.01);상술지표량조환자조간비교,NIPPV조우우대조조,차이유통계학의의(P<0.05)。결론:응용무창정압통기보조치료COPD급성가중기병례적림상료효우우상규치료,치득추엄。
Objective: To investigate the clinical efficacy of noninvasive positive pressure ventilation ( NIPPV ) therapy in treatment of acute exacerbation of Patients wiht chronic obstructive pulmonary disease ( COPD) . Methods:68 patients with COPD at acute exacerbation stage were randomly divided into control group and NIPPV group, 34 cases in each group. Control group used the conventional treatment;while NIPPV group received conventional treatment and assisted NIPPV therapy. The symptoms, signs and PaCO2 , PaO2 , pH value and other indicators of the two groups were observed. Results:Compared with those before the treatment, the symptoms and signs of the two groups were improved, and PaCO2, PaO2, and pH value were also improved (P<0. 01). Compared with control group, NIPPV group had better indicators, and the differences were significant (P<0. 05). Conclusions: NIPPV in the treatment of acute exacerbation of COPD has a better clinical efficacy than the conventional treatment, and is worthy of promotion.
