目的:探讨人乳头瘤病毒( human papillomavirus,HPV)感染型别在宫颈病变中分布情况。方法应用基因芯片技术对175例宫颈病变患者石蜡组织进行24种HPV基因型别检测。结果①检测19种基因型,HPV总感染率为45.14%(79/175),HPV单一感染与多重感染分别为74.68%和25.32%,以高危型单一感染组最高,占60.76%,其型别前四位分别是HPV16、HPV59、HPV58、HPV81亚型。②不同年龄组间以年龄≥60岁组阳性率(62.5%)最高,不同年龄段HPV感染差异无统计学意义(P>0.05)。③高危型单一感染HPV组、高危型多重感染HPV组、高低危混合型多重感染HPV组与低危型单一感染HPV组宫颈病变程度比较,差异均有统计学意义(P<0.05)。高危型单一感染HPV组与高危型多重感染HPV组、低危型多重感染HPV组、高低危混合型多重感染HPV组宫颈病变程度比较差异均无统计学意义(P>0.05);高危型多重感染HPV组与低危型多重感染HPV组比较差异无统计学意义(P>0.05)。结论高危型HPV感染是漳州地区妇女宫颈病变中常见亚型,高危型HPV感染增加了宫颈鳞癌的患病风险,而多重感染可能与宫颈病变进展无关。

목적:탐토인유두류병독( human papillomavirus,HPV)감염형별재궁경병변중분포정황。방법응용기인심편기술대175례궁경병변환자석사조직진행24충HPV기인형별검측。결과①검측19충기인형,HPV총감염솔위45.14%(79/175),HPV단일감염여다중감염분별위74.68%화25.32%,이고위형단일감염조최고,점60.76%,기형별전사위분별시HPV16、HPV59、HPV58、HPV81아형。②불동년령조간이년령≥60세조양성솔(62.5%)최고,불동년령단HPV감염차이무통계학의의(P>0.05)。③고위형단일감염HPV조、고위형다중감염HPV조、고저위혼합형다중감염HPV조여저위형단일감염HPV조궁경병변정도비교,차이균유통계학의의(P<0.05)。고위형단일감염HPV조여고위형다중감염HPV조、저위형다중감염HPV조、고저위혼합형다중감염HPV조궁경병변정도비교차이균무통계학의의(P>0.05);고위형다중감염HPV조여저위형다중감염HPV조비교차이무통계학의의(P>0.05)。결론고위형HPV감염시장주지구부녀궁경병변중상견아형,고위형HPV감염증가료궁경린암적환병풍험,이다중감염가능여궁경병변진전무관。
Objective To explore the distribution of different genotypes of human papillomavirus( HPV) infection in cervical lesions. Methods The gene chip technology was used to detect 24 types of HPV genotypes in paraffin tissues from 175 patients with cervical le-sions. Results ①Of 19 types of HPV genotypes,the total infection rate of HPV reached 45. 14%(79/175),with the infection rate of 74. 68% for single infection and 25. 32% for multiple infection. The infection rate of high risk single infection was the highest (60. 76%). The genotypes of HPV infection ranked in the top four were as followed:HPV16, HPV59,HPV58, HPV81.②The infection rate was highest(62. 5%)in the patients aged≥60 years. There was no significant difference in the HPV infection among different age patients(P>0. 05).③The cervical disease degree was significantly different between high risk single infection group,high risk multiple infection group,mixed type multiple infection group and low risk single infection group(P<0. 05),but not between high risk single in-fection group and high or low risk multiple infection groups, mixed type multiple infection group(P>0. 05). There was no significant difference between high and low risk multiple infection groups(P>0. 05). Conclusion The high-risk HPV infection is the common genotypes of HPV infection in Zhangzhou. And the high-risk HPV infection could increase the risk of squamous carcinoma of the cer-vix, while multiple infections may have nothing to do with the development of cervical lesions.
