目的:探讨糖调节受损( IGR)患者颈动脉弹性与血清炎性因子的相关性。方法应用超声多普勒中的血管回声跟踪技术,检测IGR患者颈内动脉内中膜厚度( IMT)和颈总动脉压力应变弹性系数( Eρ)、僵硬度参数(β)、动脉顺应性( AC)、脉搏波传导速度( PWVβ),同时测定血压、体质量指数、血糖、血脂、胰岛素抵抗指数、血管细胞黏附因子-1(VCAM-1)、血清细胞黏附分子-1(ICAM-1)等指标,与正常糖耐量者(NGT)进行比较。结果 IGR患者颈总动脉Eρ、β、PWVβ显著高于NGT组( P<0.05),颈动脉IMT无明显升高( P>0.05);IGR患者VCAM-1、ICAM-1显著高于NGT组(P<0.05);IGR患者VCAM-1、ICAM-1与颈总动脉Eρ、β、PWVβ呈正相关,与AC呈负相关。结论 IGR人群已存在颈动脉弹性改变和内皮功能的紊乱,应及早对IGR人群进行干预。

목적:탐토당조절수손( IGR)환자경동맥탄성여혈청염성인자적상관성。방법응용초성다보륵중적혈관회성근종기술,검측IGR환자경내동맥내중막후도( IMT)화경총동맥압력응변탄성계수( Eρ)、강경도삼수(β)、동맥순응성( AC)、맥박파전도속도( PWVβ),동시측정혈압、체질량지수、혈당、혈지、이도소저항지수、혈관세포점부인자-1(VCAM-1)、혈청세포점부분자-1(ICAM-1)등지표,여정상당내량자(NGT)진행비교。결과 IGR환자경총동맥Eρ、β、PWVβ현저고우NGT조( P<0.05),경동맥IMT무명현승고( P>0.05);IGR환자VCAM-1、ICAM-1현저고우NGT조(P<0.05);IGR환자VCAM-1、ICAM-1여경총동맥Eρ、β、PWVβ정정상관,여AC정부상관。결론 IGR인군이존재경동맥탄성개변화내피공능적문란,응급조대IGR인군진행간예。
Objective To observe the correlation of carotid artery elasticity and serum adhesion molecule in sugar reg -ulation damaged patients .Methods In all subjects , the intima-media thickness ( IMT) of carotid arteries was measured with high resolution ultrasound.The pressure-strain elastic modulus (Ep), stiffness parameter(β), arterial compliance ( AC) and pulse wave velocity ( PWVβ) were detected by ultrasonic vascular echo tracking technology ( ET) .Blood Pres-sure, body mass index, blood glucose, lipids, insulin resistance index, VCAM-1, ICAM-1 were tested.Results Com-pared with NGT group, Ep,βand PWVβin IGR were significantly higher (all P<0.05), but AC did not change obvious-ly(P<0.05).Compared with NGT group, VCAM-1, ICAM-1 in IGR group were increased significantly (all P<0.05). By correlation analysis, VCAM-1, ICAM-1 in IGR group were positively correlated with Ep,βand PWVβ(all P<0.05), and negatively correlated with AC (all P<0.05).Conclusion There have been changes of carotid artery elasticity and endothelial function disorder in the sugar regulation damaged patients .Comprehensive interventions should be carried out in IGR patients as soon as possible .
