目的:探讨2010~2012年某三甲医院肿瘤患者无菌体液感染病原菌的分布和耐药性变化情况。方法采用VITEK 2-Compact全自动鉴定及药敏分析仪对恶性肿瘤患者无菌体液进行细菌、真菌鉴定和药敏试验,采用Whonet5.6统计软件进行数据分析。结果3323株病原菌主要分离自穿刺液、血液和胆汁等标本中,占93.7%;无菌体液感染多见于胃部肿瘤科、肝胆肿瘤科和结直肠肿瘤科,占51.7%。革兰阴性菌、革兰阳性菌和真菌的构成比为48.8%、45.6%和5.6%。革兰阳性菌对万古霉素、利奈唑胺及替加环素高度敏感,革兰阴性菌对碳青霉烯类、阿米卡星及哌拉西林/他唑巴坦高度敏感,酵母菌对常用抗真菌药物的耐药率均较低。结论肿瘤患者无菌体液中分离的病原菌种类复杂,2010~2012年该院病原菌对各种抗生素的耐药率没有明显变化。

목적:탐토2010~2012년모삼갑의원종류환자무균체액감염병원균적분포화내약성변화정황。방법채용VITEK 2-Compact전자동감정급약민분석의대악성종류환자무균체액진행세균、진균감정화약민시험,채용Whonet5.6통계연건진행수거분석。결과3323주병원균주요분리자천자액、혈액화담즙등표본중,점93.7%;무균체액감염다견우위부종류과、간담종류과화결직장종류과,점51.7%。혁란음성균、혁란양성균화진균적구성비위48.8%、45.6%화5.6%。혁란양성균대만고매소、리내서알급체가배소고도민감,혁란음성균대탄청매희류、아미잡성급고랍서림/타서파탄고도민감,효모균대상용항진균약물적내약솔균교저。결론종류환자무균체액중분리적병원균충류복잡,2010~2012년해원병원균대각충항생소적내약솔몰유명현변화。
Objective To explore the distribution and drug resistance of pathogens isolated from sterile fluid of cancer patients of a hospital during the period of 2010-2012 .Methods VITEK 2-Compact automatic analysis system was used for identification and susceptibility of bacteria and fungi .Whonet5.6 was used for statistic.Results Total of 3 323 strains were identified during the period of 2010-2012 , and most of them were isolated from puncture fluid , blood, bile, etc, ac-counting for 93.7%.Sterile body fluids infection was more common in the Stomach , Hepatobiliary and Colorectal Cancer department, accounting for 51.7%.The ratio of Gram-negative bacteria, Gram-positive bacteria and fungi were 48.8%, 45.6%, 5.6%, respectively.Gram-positive bacteria showed high sensitive to Vancomycin , Linezolid and Tigecycline. Gram-negative bacteria exhibited high sensibility to Carbapenems , Amikacin and Piperacillin /Tazobactam .Drug resist-ance of Yeasts to commonly used antifungal was low .Conclusion Pathogens isolated from sterile body fluids of cancer pa-tients was complex , and the resistant rate to various antibiotics do not change significantly .
