目的：探讨不同位置的肾动脉瘤的介入治疗方法并总结其疗效。方法：2008年6月-2013年6月收治肾动脉瘤患者15例，共16个肾动脉瘤，其中肾动脉主干动脉瘤4个，肾动脉一级分支3个，肾动脉二级分支3个，肾动脉末梢分支5个。肾动脉主干动脉瘤，采用裸支架联合凝血酶方法进行治疗；位于各级分支肾动脉瘤选用弹簧圈进行填塞治疗。结果：1个肾动脉主干动脉瘤因术后复查肾动脉造影发现动脉瘤仍显影，予以补充行瘤腔内注射凝血酶治疗后，瘤腔完全闭塞；其余肾动脉瘤术后复查造影瘤腔内无造影剂充填，手术成功率100%。未见动脉瘤破裂出血及复发、增大；无相关严重并发症或死亡；尿常规、肾功能均未见明显异常。结论：采用裸支架联合凝血酶方法治疗肾动脉主干动脉瘤，可以达到保持肾动脉通畅及完全栓塞肾动脉瘤的目的；而对于肾实质内动脉瘤，选用弹簧圈进行填塞，是一种安全有效的治疗方法。

목적：탐토불동위치적신동맥류적개입치료방법병총결기료효。방법：2008년6월-2013년6월수치신동맥류환자15례，공16개신동맥류，기중신동맥주간동맥류4개，신동맥일급분지3개，신동맥이급분지3개，신동맥말소분지5개。신동맥주간동맥류，채용라지가연합응혈매방법진행치료；위우각급분지신동맥류선용탄황권진행전새치료。결과：1개신동맥주간동맥류인술후복사신동맥조영발현동맥류잉현영，여이보충행류강내주사응혈매치료후，류강완전폐새；기여신동맥류술후복사조영류강내무조영제충전，수술성공솔100%。미견동맥류파렬출혈급복발、증대；무상관엄중병발증혹사망；뇨상규、신공능균미견명현이상。결론：채용라지가연합응혈매방법치료신동맥주간동맥류，가이체도보지신동맥통창급완전전새신동맥류적목적；이대우신실질내동맥류，선용탄황권진행전새，시일충안전유효적치료방법。
Objective:To investigate the methods of interventional treatment of renal artery aneurysm in different positions and summarize the therapeutic effect.Methods:15 patients with renal artery aneurysm were selected from June 2008 to June 2013.A total of 16 renal artery aneurysm,in which 4 cases of main renal artery aneurysm,3 cases of first grade renal artery branch,and 3 cases of second grade branches of renal artery,5 cases of renal artery distal branch.Renal artery aneurysm with the treatment of bare stent combined with thrombin;renal artery aneurysm located in the branches were treated with selection of coil.Results:1 case still developing of main renal artery aneurysm found by renal artery angiography after the review aneurysms,then added for intraluminal thrombin injection after treatment,and tumor cavity occlusion;after operation the rest of renal artery aneurysm cavity without contrast agent filling when repeated angiography aneurysm,the success rate of operation was 100%.There was no bleeding and recurrence,no increase of rupture aneurysm,no serious complications or death,and the urine routine and renal function showed no significant abnormality.Conclusion:Using the bare stent combined with thrombin method in the treatment of renal artery aneurysm can achieve maintain purpose of renal artery patency and complete embolization of renal artery aneurysm.For renal artery aneurysm,spring ring packing is a safe and effective treatment method.
