目的:探讨双源Flash Spiral CT 前瞻性心电触发序列(step‐and‐shoot ,SAS)模式脑血管成像的临床应用价值。方法将60例临床怀疑脑血管性病变的患者,行前瞻性心电触发序列模式的脑血管CTA(SAS‐CTA)扫描,采集期相:应用60% R‐R间期的ECG演示模式,对SAS‐CTA做图像后处理,分析评价图像质量评分、统计辐射剂量。结果评价SAS‐CTA ,图像质量评分为4.72±0.50,平均有效剂量为(0.216±0.01)mSv ,SAS‐CTA有效辐射剂量与双能量CTA (DE‐CTA)比较降幅达到76.31%。60例临床怀疑脑血管病变患者,SAS‐CTA检出15例动脉瘤,1例动静脉畸形,1例烟雾病,25例脑血管狭窄或闭塞,18例阴性患者。结论前瞻性心电触发序列模式应用于脑血管成像,能准确诊断脑血管病变,可在保证图像质量的同时显著降低辐射剂量,有良好的临床应用价值。

목적:탐토쌍원Flash Spiral CT 전첨성심전촉발서렬(step‐and‐shoot ,SAS)모식뇌혈관성상적림상응용개치。방법장60례림상부의뇌혈관성병변적환자,행전첨성심전촉발서렬모식적뇌혈관CTA(SAS‐CTA)소묘,채집기상:응용60% R‐R간기적ECG연시모식,대SAS‐CTA주도상후처리,분석평개도상질량평분、통계복사제량。결과평개SAS‐CTA ,도상질량평분위4.72±0.50,평균유효제량위(0.216±0.01)mSv ,SAS‐CTA유효복사제량여쌍능량CTA (DE‐CTA)비교강폭체도76.31%。60례림상부의뇌혈관병변환자,SAS‐CTA검출15례동맥류,1례동정맥기형,1례연무병,25례뇌혈관협착혹폐새,18례음성환자。결론전첨성심전촉발서렬모식응용우뇌혈관성상,능준학진단뇌혈관병변,가재보증도상질량적동시현저강저복사제량,유량호적림상응용개치。
Objective To assess the clinical value of dual‐source Flash Spiral CT Prospective ECG‐triggered sequence (step‐and‐shoot ,SAS) mode in cerebral angiography .Methods 60 patients with clinically suspected cerebral vascular dis‐ease underwent SAS‐CTA scan .60% R‐R interval of the ECG presentation mode was applied for data acquisition .After image processing of SAS‐CTA ,the scores of image quality were evaluated and the radiation dose was statistically analyzed . Results When AS‐CTA was evaluated ,the image quality score was 4 .72 ± 0 .50 ,the average effective dose was (0 .216 ± 0 .01) mSv .Compared radiation dose of SAS‐CTA with that of dual‐energy CTA (DE‐CTA) ,the effective dose was de‐creased by 76 .31% .Of these 60 patients with clinically suspected cerebral vascular diseases ,15 patients were diagnosed as intracranial aneurysm ,1 patient as arteriovenous malformation (AVM ) ,1 patients as moyamoya disease ,25 patients as cerebral artery stenosis or occlusion ,and the remaining 18 patients with negative results .Conclusion Application of pro‐spective ECG triggering sequence mode in cerebral angiography can guarantee the image quality ,meanwhile ,significantly reduce the radiation dose .
