目的:探讨精氨酸对外科重症患者营养和免疫的影响。方法收集32例外科重症患者,随机分为精氨酸组(n=16例)和对照组(n=16例),精氨酸组患者在常规营养支持基础上静脉给予精氨酸20g/d,连用7d。治疗第1d与第8d分别抽血检测营养指标(血清白蛋白、前白蛋白)、细胞免疫指标(CD3+、CD4+)、体液免疫指标(IgA、IgG、IgM)。结果精氨酸组患者第8d血清白蛋白、前白蛋白、CD3+、CD4+、IgG、IgM均较第1d明显升高(P<0.05),对照组患者仅第8d血清前白蛋白均较第1d明显升高(P<0.05)。两组患者白蛋白、IgA升高程度比较,差异无统计学意义(P>0.05);前白蛋白、CD3+、CD4+、IgG、IgM升高程度比较,差异有统计学意义(P<0.05)。结论添加精氨酸的营养支持能有效提高外科重症患者免疫应答水平,改善营养状况。

목적:탐토정안산대외과중증환자영양화면역적영향。방법수집32예외과중증환자,수궤분위정안산조(n=16례)화대조조(n=16례),정안산조환자재상규영양지지기출상정맥급여정안산20g/d,련용7d。치료제1d여제8d분별추혈검측영양지표(혈청백단백、전백단백)、세포면역지표(CD3+、CD4+)、체액면역지표(IgA、IgG、IgM)。결과정안산조환자제8d혈청백단백、전백단백、CD3+、CD4+、IgG、IgM균교제1d명현승고(P<0.05),대조조환자부제8d혈청전백단백균교제1d명현승고(P<0.05)。량조환자백단백、IgA승고정도비교,차이무통계학의의(P>0.05);전백단백、CD3+、CD4+、IgG、IgM승고정도비교,차이유통계학의의(P<0.05)。결론첨가정안산적영양지지능유효제고외과중증환자면역응답수평,개선영양상황。
Objective To explore the effect of arginine on nutrition and immunity in surgical critical patients. Methods 32 patients were randomly divided into arginine group (n=16) and the control group (n=16). Arginine group patients received intra-venous arginine 20g/d for 7 days while accepting conventional nutritional support. All patients were detected about nutritional in-dexes (ALB,PA),cellular immune indexes (CD3+,CD4+) and humoral immune indexes (IgA,IgG,IgM) on the first and eighth days. Results On the eighth day after treatment,the ALB,PA,CD3+、CD4+,IgG,IgM of arginine group and the PA of control group were significantly elevated(P<0.05). The comparison of elevate degree of ALB,IgA between two groups had no significant difference (P>0.05) and the comparison of elevate degree of PA,CD3+,CD4+,IgG,IgM between two groups had significant difference (P<0.05). Conclusion Nutrition support with arginine in surgical critically patients can effectively increase the immune response level and improve nutritional status.
