目的:检测腮腺肿瘤患者唾液和血液中CA125、CEA的含量及其在腮腺肿瘤组织中的表达。方法选择腮腺良性肿瘤患者83例,腮腺恶性肿瘤患者18例,对照组为30名健康志愿者。采用化学发光法( CLIA)检测唾液及血液样本中CEA和CA125含量。采用免疫组化法检测腮腺肿瘤组织中CEA和CA125的表达。结果腮腺肿瘤患者混合唾液中CEA和CA125水平明显高于对照组。腮腺肿瘤患者与对照组血液中CEA和CA125水平无显著差异。良、恶性肿瘤患者唾液中CEA和CA125含量无显著差异。 CEA及CA125的分布特点为,混合唾液中含量最高,血液中含量最低。相关性分析显示,混合唾液中CEA与CA125呈现明显正相关性( r=0.652-0.913)。在大部分腮腺肿瘤组织中,CEA和CA125均为阳性表达,而且部分病例的肿瘤组织周围正常腺体也可见CEA及CA125的阳性表达。结论唾液中CEA和CA125水平的变化可以反映腮腺肿瘤的存在,但无法区分腮腺肿瘤的良恶性。血液中CEA和CA125不能反映腮腺肿瘤的存在。腮腺肿瘤患者的混合唾液样本中CEA与CA125呈现同步变化的趋势。唾液中CEA与CA125可能主要来源于涎腺腺体及肿瘤组织,而不是来自血液。

목적:검측시선종류환자타액화혈액중CA125、CEA적함량급기재시선종류조직중적표체。방법선택시선량성종류환자83례,시선악성종류환자18례,대조조위30명건강지원자。채용화학발광법( CLIA)검측타액급혈액양본중CEA화CA125함량。채용면역조화법검측시선종류조직중CEA화CA125적표체。결과시선종류환자혼합타액중CEA화CA125수평명현고우대조조。시선종류환자여대조조혈액중CEA화CA125수평무현저차이。량、악성종류환자타액중CEA화CA125함량무현저차이。 CEA급CA125적분포특점위,혼합타액중함량최고,혈액중함량최저。상관성분석현시,혼합타액중CEA여CA125정현명현정상관성( r=0.652-0.913)。재대부분시선종류조직중,CEA화CA125균위양성표체,이차부분병례적종류조직주위정상선체야가견CEA급CA125적양성표체。결론타액중CEA화CA125수평적변화가이반영시선종류적존재,단무법구분시선종류적량악성。혈액중CEA화CA125불능반영시선종류적존재。시선종류환자적혼합타액양본중CEA여CA125정현동보변화적추세。타액중CEA여CA125가능주요래원우연선선체급종류조직,이불시래자혈액。
Objective To determine the levels of CEA and CA125 in saliva and serum of patients with parotid tumor and their expression. Methods A total of 101 patients with unilateral parotid tumor and 30 healthy subjects were included in this study. Concentrations of CEA and CA125 were measured with chemiluminescence immunoassay ( CLIA) , and their expression in tumor tissues from 18 patients were examined by immunohistochemistry. Results Levels of CEA and CA125 in whole saliva were significantly higher in patients than in the controls, but the levels of CEA and CA125 in serum showed no significant difference among control and patients with benign and malignant tumors. There was no significant difference in saliva level between benign and malignant tumors. The highest levels of CEA and CA125 were found in saliva, while the lowest in serum. Analysis showed that positive correlation was found between CEA and CA125 in whole saliva. CEA and CA125 were expressed in most of parotid tumor tissue and in the acinus adjacent to tumor tissue of some cases. Conclusion The elevated levels of CEA and CA125 in saliva could reflect the occurrence of parotid tumor, but the levels of CEA and CA125 in serum showed no significant difference between patients and the control. It is possible that CEA and CA125 in saliva may originate from parotid tumor and gland instead of serum.
