目的：探讨胸苷磷酸化酶（ TP）在胃癌组织中的表达、微血管密度（ MVD）与胃癌临床病理特征及预后间的关系。方法采用免疫组化 SP法检测77例胃癌组织及25例癌旁正常胃黏膜组织中 TP的表达，选择CD34作为血管内皮细胞标记物，对CD34阳性血管进行MVD计数。结果 TP和MVD在胃癌组织中的表达率分别为59．74％和（34．99±9．46）％，明显高于在癌旁正常组织中的表达12．0％和（12．65±4．00）％，差异有统计学意义（P＜0．05）。 TP和MVD在胃癌组织中的表达与淋巴结转移、浸润深度及临床分期有关（P＜0．05），而与肿瘤大小、分化程度及肿瘤大体分型无关。 TP阴性表达者的5年总生存期明显高于阳性表达者，差异有统计学意义（P＜0．05）。 MVD≤33组的胃癌患者5年总生存期明显优于MVD＞33组，差异有统计学意义（P＜0．05）；经相关分析，TP阳性表达率与MVD呈正相关（P＜0．05）。结论 TP在胃癌组织中存在高表达，且其表达和MVD呈正相关，并与胃癌的生长、浸润、转移及预后有关。
목적：탐토흉감린산화매（ TP）재위암조직중적표체、미혈관밀도（ MVD）여위암림상병리특정급예후간적관계。방법채용면역조화 SP법검측77례위암조직급25례암방정상위점막조직중 TP적표체，선택CD34작위혈관내피세포표기물，대CD34양성혈관진행MVD계수。결과 TP화MVD재위암조직중적표체솔분별위59．74％화（34．99±9．46）％，명현고우재암방정상조직중적표체12．0％화（12．65±4．00）％，차이유통계학의의（P＜0．05）。 TP화MVD재위암조직중적표체여림파결전이、침윤심도급림상분기유관（P＜0．05），이여종류대소、분화정도급종류대체분형무관。 TP음성표체자적5년총생존기명현고우양성표체자，차이유통계학의의（P＜0．05）。 MVD≤33조적위암환자5년총생존기명현우우MVD＞33조，차이유통계학의의（P＜0．05）；경상관분석，TP양성표체솔여MVD정정상관（P＜0．05）。결론 TP재위암조직중존재고표체，차기표체화MVD정정상관，병여위암적생장、침윤、전이급예후유관。
Objective To detect the expression of TP and correlation of microvessel density ( MVD) in gastric carcinoma and to discuss the relationship between the clinical and pathological characteristics and prognosis of gastric cancer. Methods The expressions of TP,MVD were studied by immunohistochemistry in 77 gastric carcinoma specimens and 25 normal tissues. CD34 was chosen as the vascular endothelial cell markers to count MVD. Results The rate of positive expression of TP and MVD in gastric cancer tissue was 59. 74% and 34. 99 ± 9. 46,respectively,which were significantly higher than those in neighboring gastric cancer tissue that were 12. 0%and 12. 65 ± 4. 00,respectively. All of the differences had statistically significant (all,P<0. 05). The positive expression rates of TP and MVD were significantly correlated with depth of invasion,lymph node metastasis and clinical stage in gastrointestinal carcinoma patients (P<0. 05),but were not correlated with differentiation,tumor size and vascular of invasion (P>0. 05). The five-year overall survival was higher in patients with tumors negative for TP than in those with tumors positive for TP;the five-year overall survival was lower in patients with tumors having high MVD score than in patients with tumors having low MVD score (P<0. 01). In gastric carcinoma,the expression had a positive correlation with TP and MVD (P<0. 01). Conclusion TP and MVD are related to the growth,infiltration, metastasis and prognosis of gastric carcinoma.
