目的:探讨COPD患者营养状况与病情分级及机械通气风险间的关系。方法测定159例COPD患者营养指标,记录肺功能FEV1/FVC、FEV1 pred%、近一年内急性发作次数、mMRC分级及机械通气情况,并进行分组。结果 COPD患者BMI、白蛋白和淋巴细胞计数的营养不良发生率分别为51.9%、42.8%和48.4%。营养指标中BMI、白蛋白、前蛋白和淋巴细胞计数与FEV1/FVC、FEV1 pred%均有相关性,且与病情严重程度相关(P<0.05),球蛋白与FEV1/FVC及FEV1 pred%无相关性。急性加重高风险组营养较低风险组差(P<0.05)。机械通气组营养差于非机械通气组(P<0.05)。结论 COPD患者营养不良发生率高,且营养状态与肺功能进展、急性加重及机械通气风险相关。

목적:탐토COPD환자영양상황여병정분급급궤계통기풍험간적관계。방법측정159례COPD환자영양지표,기록폐공능FEV1/FVC、FEV1 pred%、근일년내급성발작차수、mMRC분급급궤계통기정황,병진행분조。결과 COPD환자BMI、백단백화림파세포계수적영양불량발생솔분별위51.9%、42.8%화48.4%。영양지표중BMI、백단백、전단백화림파세포계수여FEV1/FVC、FEV1 pred%균유상관성,차여병정엄중정도상관(P<0.05),구단백여FEV1/FVC급FEV1 pred%무상관성。급성가중고풍험조영양교저풍험조차(P<0.05)。궤계통기조영양차우비궤계통기조(P<0.05)。결론 COPD환자영양불량발생솔고,차영양상태여폐공능진전、급성가중급궤계통기풍험상관。
Objective To study the correlation of nutritional indicators to the risk of mechanical ventilation and disease classification in patients with chronic obstructive pulmonary disease (COPD).Methods Their nutri-tional parameters,acute episodes,mMRC classification,mechanical ventilation,FEV1 /FVC and FEV1 pred% were observed and recorded.Results The malnutrition rates of BMI,albumin and lymphocyte count in COPD patients were 52.2%,42.8% and 48.4% respectively.BMI,albumin,former protein and lymphocyte count were correlate with FEV1 /FVC and FEV1 pred%,as well as associated with disease severity (P<0.05 ).There was no correlation between globulin and the levels of FEV1 /FVC and FEV1 pred%.The nutrition status was worse in the high-risk group than in the low-risk group (P<0.05 ),and it was also worse in the mechanical ventilation group than in the non-me-chanical ventilation group (P<0.05 ).Conclusion The incidence of malnutrition in COPD patients is high,which shows an effect on their prognosis,lung function and mechanical ventilation.
