目的:研究实时荧光核酸恒温扩增检测技术(SAT)在肺结核诊断中的应用价值。方法采用SAT法、金胺O染色法、BACTEC MGIT960法和酸性罗氏培养基法对342例痰标本进行检测。结果 SAT法的灵敏度、阴性预测值、符合率和诊断指数比另外三种方法高,差异有统计学意义(P值均<0.05)。SAT法和BACTEC MGIT 960法联合检测,灵敏度、阴性预测值、符合率和诊断指数均高于单一SAT法,差异有统计学意义(Z值=2.25-3.11,P值均<0.05)。结论 SAT法是灵敏度高、特异性强的结核检测方法,在结核诊断中有重要价值。联合BACTEC MGIT 960方法可有效提高灵敏度、阴性预测值及诊断指数,推荐临床使用。

Objective To evaluate the clinical application of simultaneous amplification and testing (SAT) in the diagnosis of pulmonary tuberculosis.Methods 342 cases of sputum specimens were detected by simultaneous amplification and testing,fluorescence staining smear method,BACTEC MGIT 960 test method and acid L-G medium method respectively.Results The sensitivity,PV-,accuracy and diagnostic index of SAT method were higher than the other methods (Z=0.34-5.91 ,P<0.05 ).When the two methods of SAT and BACTEC MGIT 960 were com-bined,the sensitivity,PV-,accuracy and diagnostic index became higher than single SAT did (Z=2.25 -3.1 1 , P<0.05 ).Conclusion The SAT method has high sensitivity and specificity in the diagnosis of mycobacterium tu-berculosis,which shows higher sensitivity and specificity while combined with BACTEC MGIT960 method.
