目的研究狼疮性肾炎(LN)患者外周血辅助性T细胞1 (Th1)、辅助性T细胞17(Th17)和辅助性T细胞22(Th22)及其细胞因子干扰素-γ(IFN-γ)、白细胞介素17(IL-17)和白细胞介素22(IL-17)的表达情况及其临床意义.方法流式细胞术检测20例LN患者组和20例健康对照组外周血中Th1、Th17和Th22细胞比例;酶联免疫吸附试验(ELISA)检测血清中相关细胞因子IFN-γ、IL-17及IL-22水平,并进行相关性分析.结果相较健康对照组,LN组外周血中Th17细胞比例增高(P＜0.05),Th22细胞比例减低(P＜0.05),而Th1细胞比例无差异(P＞ 0.05);LN组血清中IL-17水平增高(P＜0.05),IL-22水平减低(P＜0.05),而IFN-γ水平无差异(P＜0.05);Th17细胞比例分别与血清IL-17、系统性红斑狼疮疾病活动指数(SLEDAI)呈正相关(r=0.721,P＜0.05; r=0.225,P＜0.05),而Th22细胞比例与血清IL-22水平呈正相关(r=0.489,P＜0.05)与SLEDAI评分无相关性(r=0.241,P＞0.05).结论 Th17及Th22在LN患者中异常表达可能导致患者自身免疫功能紊乱,促进疾病病理损伤.
목적 연구랑창성신염(LN)환자외주혈보조성T세포1 (Th1)、보조성T세포17(Th17)화보조성T세포22(Th22)급기세포인자간우소-γ(IFN-γ)、백세포개소17(IL-17)화백세포개소22(IL-17)적표체정황급기림상의의.방법 류식세포술검측20례LN환자조화20례건강대조조외주혈중Th1、Th17화Th22세포비례;매련면역흡부시험(ELISA)검측혈청중상관세포인자IFN-γ、IL-17급IL-22수평,병진행상관성분석.결과 상교건강대조조,LN조외주혈중Th17세포비례증고(P＜0.05),Th22세포비례감저(P＜0.05),이Th1세포비례무차이(P＞ 0.05);LN조혈청중IL-17수평증고(P＜0.05),IL-22수평감저(P＜0.05),이IFN-γ수평무차이(P＜0.05);Th17세포비례분별여혈청IL-17、계통성홍반랑창질병활동지수(SLEDAI)정정상관(r=0.721,P＜0.05; r=0.225,P＜0.05),이Th22세포비례여혈청IL-22수평정정상관(r=0.489,P＜0.05)여SLEDAI평분무상관성(r=0.241,P＞0.05).결론 Th17급Th22재LN환자중이상표체가능도치환자자신면역공능문란,촉진질병병리손상.
Objective To investigate the expression of T-helper (Th)1,Th17 and Th22 cells in peripheral blood and cytokines in serum of patients with lupus nephritis (LN) and its clinical significance.Methods The proportion of Th1,Th17 and Th22 cells in peripheral blood of 20 LN patients and 20 healthy controls were evaluated by flow cytometry; the concentration of cytokines including interferon (IFN)-γ,interleukin (IL)-17 and IL-22 were measured by ELISA.The correlation between proportion of Th 17 and Th22 cells and IL-17,IL-22,systemic lupus erythematosus disease activity index (SLEDAI) were analyzed.Results The proportion of Th17 cells increased (P ＜ 0.05),the proportion of Th22 cells decreased (P ＜ 0.05),the proportion of Th 1 cells did not change too much (P ＞ 0.05) in LN patients compared with healthy controls; the concentration of serum IL-17 increased (P ＜ 0.05),the concentration of serum IL-22 decreased (P ＜ 0.05),the concentration of serum IFN-γ did not change too much (P ＞ 0.05) in LN patients compared with healthy controls.The proportion of Thl7 cells correlated positively with concentration of serum IL-17 and SLEDAI in LN patients (r =0.721,P ＜ 0.05; r =0.225,P ＜ 0.05).There was a positive correlation between the proportion of Th22 cells and concentration of serum IL-22 (r =0.489,P ＜ 0.05),no statistical correlation between the proportion of Th22 cells and SLEDAI (r =0.241,P ＞ 0.05).Conclusion The abnormal expression of Th 17 and Th22 may play an important role in LN disease pathogenicity.
