目的 探讨颅脑损伤(TBI)后早期凝血功能、血白细胞(WBC)及血糖水平的变化与损伤程度及预后的关系.方法 依据格拉斯哥昏迷评分(GCS)将126例颅脑损伤患者分为轻中型、重型损伤两组;1个月后随访依据牛津残障评分(OHS)分为预后不良组和预后良好组,测定其入院时的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、血小板(PLT)、WBC和血糖的水平.选取同期30例健康体检者作为对照组.比较各组PT、APTT、FIB、PLT、WBC及血糖水平.结果 轻中型TBI组、重型TBI组PT、APTT、WBC和血糖水平明显高于对照组,而PLT和FIB水平明显低于对照组(P<0.05);随着损伤程度加重,PT、APTT、WBC和血糖水平逐渐升高,而PLT和FIB水平逐渐降低(P<0.05);与预后良好组相比,TBI预后不良组PT、APTT、WBC和血糖水平明显升高,PLT、FIB水平和GCS评分明显降低(P<0.05);PT、APTT、WBC、血糖分别与GCS、OHS评分呈负相关,PLT、FIB与GCS、OHS评分呈正相关(P<0.05).结论 TBI患者早期凝血功能、血WBC及血糖与颅脑损伤程度和预后密切相关,可初步用于病情评估和预后判断.

목적 탐토로뇌손상(TBI)후조기응혈공능、혈백세포(WBC)급혈당수평적변화여손상정도급예후적관계.방법 의거격랍사가혼미평분(GCS)장126례로뇌손상환자분위경중형、중형손상량조;1개월후수방의거우진잔장평분(OHS)분위예후불량조화예후량호조,측정기입원시적응혈매원시간(PT)、활화부분응혈활매시간(APTT)、섬유단백원(FIB)、혈소판(PLT)、WBC화혈당적수평.선취동기30례건강체검자작위대조조.비교각조PT、APTT、FIB、PLT、WBC급혈당수평.결과 경중형TBI조、중형TBI조PT、APTT、WBC화혈당수평명현고우대조조,이PLT화FIB수평명현저우대조조(P<0.05);수착손상정도가중,PT、APTT、WBC화혈당수평축점승고,이PLT화FIB수평축점강저(P<0.05);여예후량호조상비,TBI예후불량조PT、APTT、WBC화혈당수평명현승고,PLT、FIB수평화GCS평분명현강저(P<0.05);PT、APTT、WBC、혈당분별여GCS、OHS평분정부상관,PLT、FIB여GCS、OHS평분정정상관(P<0.05).결론 TBI환자조기응혈공능、혈WBC급혈당여로뇌손상정도화예후밀절상관,가초보용우병정평고화예후판단.
Objective To explore the correlation between early coagulation function,white blood cells (WBC),blood glucose,and the severity and prognosis in patients with traumatic brain injury (TBI).Methods A total of 126 cases of traumatic brain injury was divided into two groups (mild-moderate injury; and severe injury) based on the Glasgow Coma Scale (GCS).After one month,Oxford handicap scale was employed to assess the prognosis of all the TBI patients,based on that they were divided into the favorable prognosis group and poor prognosis group.Prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen (FIB),platelet (PLT),WBC,and blood glucose levels were measured after admission.Thirty cases of health physical examination persons were selected as control group at the same period.The levels of PT,APTT,FIB,PLT,WBC,and blood glucose were compared among different groups.Results The levels of APTT,PT,WBC,and blood glucose in mild-moderate TBI group and severe TBI group were significantly higher than those in the control group,while the levels of PLT and FIB were significantly lower (P < 0.05).The levels of PT,APTT,WBC,and blood glucose were progressively elevated with increasing degree of injury,while the levels of PLT and FIB were decreased (P <0.05).Compared to good prognosis group,the levels of PT,APTT,WBC,and blood glucose were gradually increased in poor prognosis group,while PLT,FIB and GCS score were decreased (P <0.05).The levels of PT,APTT,WBC,and blood glucose were negatively correlated with GCS score and OHS score,while FIB and PLT were positively correlated (P < 0.05).Conclusions Early coagulation function,WBC,and blood glucose in TBI patients were closely related to brain injury degree and its prognosis,which might be used in evaluation of the severity and prognosis.
