目的 探讨不同类型献血者不规则抗体检出率及分布情况,提高临床输血的安全性.方法 选择2004年1月至2012年12月于东营市中心血站无偿献血的175 587例献血者为研究对象.其中,男性献血者为121 682例,女性为53 905例.年龄为18～55岁,中位年龄为(33.2±6.5)岁.所有献血者均符合《献血者健康检查要求》(GB18467-2001或GB18467-2011)要求.所有血液的采集均取得献血者知情同意,并在知情同意一栏签字后献血.采用盐水凝集法对献血者不规则抗体进行筛查,并采用抗人球蛋白间接法和筛查谱细胞法对不规则抗体阳性结果进行确认.结果 本组175 587例无偿献血者中,共检出103例献血者存在不规则抗体,不规则抗体总检出率为0.059％.不同血型献血者不规则抗体检出率如下:O型为0.076％,A型为0.051％,B型为0.052％,AB型为0.060％;不同血型献血者不规则抗体检出率比较,差异无统计学意义(x2 =3.34,P＞0.05).不同年龄段献血者不规则抗体检出率如下:18～25岁为0.029％,＞25～35岁为0.062％,＞35～45岁为0.103％,＞45～55岁为0.134％;不同年龄段献血者不规则抗体检出率比较,差异有统计学意义(x2 =31.38,P＜0.01).不同性别献血者不规则抗体检出率如下:女性为o.082％(44/53 905),男性为0.048％(59/121 682),女性献血者不规则抗体检出率较男性高,且二者比较,差异有统计学意义(x2 =7.00,P＜0.01).本组18例献血者不规则抗体的抗体特异性鉴定结果示,Rh系统为9例(50.0％,9/18),MNS系统为7例(38.9％,7/18).结论 不规则抗体检出率随献血者年龄的增大而升高.不规则抗体以抗-E、抗-M抗体多见.35岁以上女性受血者应注意不规则抗体的筛查,以提高临床输血的安全性.

Objective To explore irregular antibody detection rate and distribution of different types of blood donors,in order to improve the clinical transfusion safety.Methods From January 2004 to December 2012,a total of 175 587 cases of blood donors in Blood Center of Dongying were included in this study.Male donors were 121 682 cases,and female were 53 905 cases.Donors' ages were 18 to 55 years old,and average ages were (33.246.5) years old.All donors were conformed by the "Requirements of Donor Health Examination" (GB18467-2001 or GB18467-2011).Informed consent was obtained from all donors before blood collection.Saline agglutination method was used to screen irregular antibodies of blood donors.Indirect anti globulin experiment and spectrum cell screening method were used to confirm positive results.Results Among these 175 587 cases of blood donors,103 cases were detected out irregular antibodies,with the rate of 0.059 ％.The irregular antibody detection rate in different bloods donors were 0.076％ in O blood type donors,0.051％ in A blood type,0.052％ in B blood type,and 0.060％ in AB blood type,respectively.Irregular antibody detection rate had no statistically significant difference among different blood types donors (x2 =3.34,P＞0.05).The irregular antibody detection rate in different ages donors were 0.029％ in 18-25 years old,0.062％ in ＞25-35 years old,0.103％ in ＞35-45 years old and 0.134％ in ＞ 45-55 years old,respectively.The difference of irregular antibody detection rate had statistically significant among different ages donors (x2 =31.38,P＜0.01).Women's irregular antibody detection rate was 0.082％,which was higher than 0.048％ of men (x2=7.00,P＜0.01).There were 9 cases in Rh blood group system (50.0％,9/18) and 7 cases in MNS blood group system in 18 donors who were detected out irregular antibody.Conclusions With the increasing of blood donors' age,the irregular antibody detection rate increases.Anti-E and anti-M irregular antibodies are more common.We should pay more attention to irregular antibody screening of over 35 years old female recipients,in order to improve the safety of clinical blood transfusion.
