目的 探讨组织多普勒成像在矽肺并发慢性肺源性心脏病患者右心室肥大的诊断价值.方法 将本院50例矽肺并发慢性肺源性心脏病患者,分别行常规心电图和组织多普勒成像检查,比较两种方法右心室肥大检出率.结果 入组的50例矽肺并发慢性肺源性心脏病患者经心电图检查,发现右心室肥大患者19例,检出率为38.0%,而组织多普勒成像技术检查发现右心室肥大患者29例,检出率为58.0%,组织多普勒成像技术对矽肺并发慢性肺源性心脏病患者右心室肥大检出率显著高于心电图,差异有统计学意义(x2=4.006,P=0.036).结论 多普勒超声心动图更有利于矽肺并发慢性肺源性心脏病右心室心肌肥大的诊断,可为矽肺合并肺心病的临床治疗提供可靠的诊断依据.

Objective To investigate the value of tissue Doppler imaging (TDI) in the diagnosis of right ventricular hypertrophy in patients with silicosis complicated by chronic pulmonary heart disease.Methods A total of 50 cases of silicosis complicated by chronic pulmonary heart disease in our hospital underwent conventional electrocardiography (ECG) and TDI.The detection rates for right ventricular hypertrophy by two methods were compared.Results Of 50 cases of silicosis complicated by chronic pulmonary heart disease,19 were diagnosed with right ventricular hypertrophy by ECG,with a detection rate of 38.0%; 29 were diagnosed with right ventricular hypertrophy by TDI,with a detection rate off 58.0%.Statistical analysis suggested that TDI leads to a significantly higher detection rate for right ventricular hypertrophy in patients with silicosis complicated by chronic pulmonary heart disease (x2=4.006,P=0.036).Conclusion Both TDI and ECG can be used for detecting right ventricular hypertrophy in patients with silicosis complicated by chronic pulmonary heart disease,but the detection rate is higher when TDI is employed.In addition,ECG cannot directly reflect the increase in pulmonary artery pressure.Therefore,TDI is more suitable for the diagnosis of right ventricular hypertrophy in patients with silicosis complicated by chronic pulmonary heart disease and provides a strong diagnostic basis for the clinical treatment of silicosis complicated by pulmonary heart disease.
