目的 应用静息态功能磁共振成像(rs-fMRI)技术探讨难治性颞叶癫痫(ITLE)患者海马功能连接的变化情况. 方法 选择自2013年8月至2014年5月安徽医科大学附属省立医院神经外科收治的ITLE患者18例(ITLE组)及同期招募的18例正常被试者(对照组)进行rs-fMRI数据采集,分别以左、右侧海马为感兴趣区与全脑体素进行功能连接分析,比较ITLE组和对照组海马功能连接的差异. 结果 以左侧海马为感兴趣区进行功能连接分析时,ITLE组与右侧楔叶的功能连接显著高于对照组,与双侧尾状核、右侧隔区及右侧直回的功能连接显著低于对照组,差异均有统计学意义(P<0.05);以右侧海马为感兴趣区进行功能连接分析,ITLE组与双侧楔叶、双侧舌回及左侧距状回的功能连接显著高于对照组,与双侧颞上回、右侧颞中回、左侧缘上回、左侧中央前回及左侧中央后回的功能连接显著低于对照组,差异均有统计学意义(P<0.05). 结论 rs-fMRI技术可以发现ITLE患者海马功能连接存在显著异常,提示海马是“癫痫网络”的重要组成部分.

목적 응용정식태공능자공진성상(rs-fMRI)기술탐토난치성섭협전간(ITLE)환자해마공능련접적변화정황. 방법 선택자2013년8월지2014년5월안휘의과대학부속성립의원신경외과수치적ITLE환자18례(ITLE조)급동기초모적18례정상피시자(대조조)진행rs-fMRI수거채집,분별이좌、우측해마위감흥취구여전뇌체소진행공능련접분석,비교ITLE조화대조조해마공능련접적차이. 결과 이좌측해마위감흥취구진행공능련접분석시,ITLE조여우측설협적공능련접현저고우대조조,여쌍측미상핵、우측격구급우측직회적공능련접현저저우대조조,차이균유통계학의의(P<0.05);이우측해마위감흥취구진행공능련접분석,ITLE조여쌍측설협、쌍측설회급좌측거상회적공능련접현저고우대조조,여쌍측섭상회、우측섭중회、좌측연상회、좌측중앙전회급좌측중앙후회적공능련접현저저우대조조,차이균유통계학의의(P<0.05). 결론 rs-fMRI기술가이발현ITLE환자해마공능련접존재현저이상,제시해마시“전간망락”적중요조성부분.
Objective To explore the alterations of hippocampal functional connectivity in patients with intractable temporal lobe epilepsy (ITLE) with resting state functional magnetic resonance imaging (rs-fMRI).Methods Eighteen patients with ITLE,admitted to our hospital from August 2013 to May 2014,and 18 healthy controls were chosen in our study; the fMRI data of the two groups were obtained; analysis of the resting state brain functional connectivity was performed by choosing the left and right hippocampus as the regions of interest (ROIs),and then,the differences of hippocampal functional connectivity were compared between ITLE group and healthy control group.Results When the left hippocampus was selected as ROI,significantly enhanced functional connectivity in the ITLE group was observed in the right cuneus,significantly weakened functional connectivity in the ITLE group was observed in the bilateral caudate nucleus,bilateral septal area and right gyrus rectus,as compared with those in the healthy control group (P<0.05); when the right hippocampus was selected as ROI,significantly enhanced functional connectivity in the ITLE group was observed in the bilateral cuneus,bilateral lingual gyrus,and left calcarine,significantly weakened functional connectivity in the ITLE group was observed in the bilateral superior temporal gyrus,right middle temporal gyrus,left supramarginal gyrus,left precentral gyrus and left postcentral gyrus,as compared with those in the healthy control group (P<0.05).Conclusion Hippocampal dysfunction of patients with ITLE could be found based on rs-fMRI,which prompts that the hippocampus plays an important role in the epilepsy network of patients with ITLE.
