目的 介绍应用游离第二足趾胫侧复合组织瓣修复再造手指指腹缺损的方法.方法 2011年6月至2013年6月,对24例25指手指指腹软组织缺损的患者采用游离第二足趾胫侧复合组织瓣修复.皮瓣以第二足趾胫侧趾底动脉为供血动脉,切取面积为1.8cm×2.8 cm~2.8 cm×4.5 cm.供区取全厚皮片植皮加压包扎.结果 术后24例25指再造指腹全部存活,经8个月至3年的随访(平均12个月),再造指腹外观良好,无臃肿,质地佳,两点分辨觉为5~11 mm.供区足趾植皮处稍有色素沉着,行走无影响.结论 游离第二足趾胫侧皮瓣再造指腹是修复手指指腹缺损的理想治疗方法.

Objective To introduce the surgical techniques of free transfer of second toe tibial skin flap for reconstruction of finger pulp defect.Methods From June 2011 to June 2013,24 patients with finger pulp defects of 25 fingers were treated by transfer of second toe tibial composite flap.The skin flaps used the tibial plantar digital proper artery of the second toe as the feeding vessel.The size of the skin flaps varied from 1.8 cm × 2.8 cm to 2.8 cm × 4.5 cm.The donor sites were covered by full thickness skin grafts.Results All the flaps transferred to reconstruct the 25 finger pulps in 24 patients survived uneventfully.Postoperative follow-up ranged from 8 months to 3 years,with an average of 12 months.The texture and shape of the reconstructed pulps were satisfactory.Two-point discrimination of the flaps (pulps) returned to 5 to 11 mm.Slight pigmentation of the skin grafts was noted at the donor toes.There was no impairment of walking.Conclusion Second toe tibial skin flap transfer is an ideal method to reconstruct finger pulp defects.
