目的探讨肾移植围手术期检测CMV-PP65抗原的临床意义.方法选取我院肾移植患者100例,调查统计全部患者围手术期CMV-PP65抗原及抗CMV-IgM抗体检测结果,比较两项检测指标的敏感性及特异性.结果 CMV-PP65抗原检测的敏感性明显高于抗CMV-IgM抗体检测,两者比较差异有统计学意义(P＜0.05);两种检测方法的特异性无统计学意义(P＞0.05),但是CMV-PP65抗原检测出现阳性结果的平均时间明显小于抗CMV-IgM抗体检测出现阳性结果的平均时间,两者比较差异有统计学意义(P＜0.05).结论肾移植患者围手术期检测CMV-PP65抗原对于早期诊断CMV病具有较高的敏感性,对肾移植术后CMV病的诊断和早期抗病毒治疗具有重要的临床意义,值得推广应用.

Objective To discuss the significance of CMV-PP65 antigen detection in perioperative period of kidney transplant.Methods 100 cases of kidney transplant patients were selected from our hospital,investigation and statistics about the perioperative period CMV-PP65 antigen and anti-CMV-IgM antigen detection results of all patients were made to compare the sensitivity and specificity of the two detected indexes.Results The sensitivity of CMV-PP65 antigen detection was obviously higher than that of anti-CMV-IgM antigen detection,there was statistically significant difference between the two groups (P ＜ 0.05).The specificity of the two detection methods was no obvious statistical significance(P ＞ 0.05).But the CMV-PP65 antigen detection indicated that the average time of positive result was obviously less than that of the anti-CMV-IgM antigen,and there was statistically significant difference between the two groups (P ＜ 0.05).Conclusion In kidney transplant perioerative period,the CMV-PP65 antigen detection is of high sensitivity to the early diagnosis of CMV disease and it is of important clinical significance to the diagnosis of CMV disease after post-renal transplantation and early stage antiviral therapy,which is worthy of popularization and application by us.
