目的 观察射频保脾及脾切除术后机体免疫功能的改变.方法 使用随机分组的方法将实验用长白猪分为正常对照组(10头)、射频保脾组(10头)、脾切除组(10头).分别于术后1、4、12、24周采静脉血检测机体免疫功能指标:使用酶联免疫吸附试验(ELISA)法检测血清IgM、IgG、C3、C4水平;使用流式细胞仪检测外周血T淋巴细胞CD4+、CD8+比例.结果 在体液免疫方面,IgM水平在射频保脾术后无明显变化,而脾切除组术后IgM水平显著降低;IgG、C3、C4血浆浓度以及CD4+、CD8+T淋巴细胞比例在术后监测中,各组差异均无统计学意义(P>0.05).结论 射频保脾对机体短期内体液免疫功能无明显损害,脾切除术后对机体短期体液免疫功能有损害;而无论是射频保脾还是脾脏切除对机体远期体液免疫功能及细胞免疫功能可能无明显影响.

Objective To evaluate the difference of immune function between normal control group,RF spleen preserving group (experimental group) and splenectomy group (experimental group),to explore the immune function of spleen preserving and after splenectomy changes,to provide some experimental basis for the clinical development of the spleen preserving operation.Methods Landrace pigs were divided into normal control group (10 cases),RF spleen preserving group (10 cases),spleen resection group (10 cases) by randomized.We detected immune function indexes respectively after 1 weeks,4 weeks,12 weeks,24 weeks in venous blood.We detected serum IgM,IgG,C3,C4 by enzyme linked immunosorbent assay (ELISA) method; T lymphocyte CD4 + and CD8 + ratio by blood flow cytometry.Results There was no obvious changes of IgM levels in RF spleen preserving group.IgM level decreased significantly after spleen resection.The IgG,C3,C4 and the percentage of CD4 + and CD8 + T lymphocytes showed no significant differences between the groups.Conclusion Compared with splenectomy,RF spleen preserving has no damage to short term humoral immunity.There was no effect on the long-term humoral immunity and cellular immune functionmight after both RF spleen preserving and splenectomy.
