目的 研究三维可视化技术在闭合性肝外伤诊治中的临床应用价值.方法 选择2010年6月至2014年6月20例严重肝破裂患者,其中男17例,女3例,年龄19~61岁,平均年龄33.2岁.应用腹部医学图像三维可视化软件对患者肝脏损伤情况进行三维重建,指导闭合性肝外伤的诊治.结果 本组20例患者经三维重建,观察患者出血范围、已形成血肿大小、腹部血管损伤情况,个体化决定治疗方案,患者经治疗恢复良好,住院期间无并发症发生.结论 三维可视化技术可以为闭合性肝外伤提供清晰、可视化的诊断参考,对患者个体化治疗决策的制定具有积极作用.

목적 연구삼유가시화기술재폐합성간외상진치중적림상응용개치.방법 선택2010년6월지2014년6월20례엄중간파렬환자,기중남17례,녀3례,년령19~61세,평균년령33.2세.응용복부의학도상삼유가시화연건대환자간장손상정황진행삼유중건,지도폐합성간외상적진치.결과 본조20례환자경삼유중건,관찰환자출혈범위、이형성혈종대소、복부혈관손상정황,개체화결정치료방안,환자경치료회복량호,주원기간무병발증발생.결론 삼유가시화기술가이위폐합성간외상제공청석、가시화적진단삼고,대환자개체화치료결책적제정구유적겁작용.
Objective To study the application value of the three-dimensional visualization technology in diagnosis and treatment of patients with blunt hepatic trauma.Methods A total of 20 patients with severe liver rupture were retrospectively summarized and analyzed during June 2010 and June 2014 including 17 males and 3 females,with their ages ranging from 19 to 61 and a mean of 33.2 years.The clinical data of 64-slice multidetector CT was introduced into Medical Image Three-Dimensional Visualization System (MI-3DVS) for 3-dimensional reconstruction.Based on the reconstructed 3-dimensional model,preoperative procedures planning was formulated which guiding the diagnosis and treatment of blunt hepatic trauma.Results The abdominal 3D models could clearly display the area of the bleeding livers,the size of the hematomas,as well as the injury condition of abdominal vessels,which contributed to the formulation of individualized surgical plans.The preoperative planning guided by MI-3DVS was in line with the intraoperative findings.No postoperative complications occurred in all 20 patients.Conclusion MI-3DVS could provide accurately diagnosis on patients with blunt hepatic trauma.It could also play a positive role to determine the preoperative surgical plans.
