目的评估经阴道超声(TVS)诊断盆腔子宫内膜异位症的价值.方法选择在潍坊市妇幼保健院就诊,疑为盆腔子宫内膜异位症的134例患者,所有病例经阴道超声检查后行腹腔镜手术.结果 96例患者确诊为子宫内膜异位症.TVS诊断1处异位病灶的敏感性为82.4％,而2处及以上异位病灶的敏感性为54.4％,差异有统计学意义(P＜0.05).TVS诊断卵巢子宫内膜异位囊肿的敏感性最高,而诊断盆壁、子宫骶韧带的异位病灶的敏感性较低.TVS诊断卵巢异位囊肿与其他部位的异位病灶的敏感性比较,差异有统计学意义(P＜0.01).TVS与腹腔镜比较诊断卵巢粘连与子宫直肠陷凹Kappa值分别为0.70、0.74,有较好的一致性.结论 TVS诊断盆腔子宫内膜异位症的特异性较高,其诊断的准确率受异位病灶的位置及数量影响.

Objective To evaluate the value of transvaginal ultrasound scanning(TVS) on the diagnosis of pelvic endometriosis.Methods In our hospital,134 cases of suspected pelvic endometriosis were chosen,and all patients underwent laparoscopic operation by transvaginal ultrasonography.Results The 96 women were confirmed with pelvic endometriosis by laparoscopy.The sensibility of pre-operative TVS for single location was 82.4％,while the sensibility of pre-operative TVS for two and above locations was 54.4％,and the differences were statistically significant (P ＜ 0.05).Sensitivity of pre-operative TVS diagnosis was the highest for endometriosis of ovary and was lower for uterosacral ligaments and pelvic side wall.There were significant differences(P ＜0.01).Kappa values of TVS and laparoscopic for diagnosis of ovarian adhesions and recto uterine cavity were 0.70 and 0.74 with good consistency.Conclusions Transvaginal ultrasound scanning has high specificity for the diagnosis of pelvic endometriotic.The accuracy of ultrasound diagnosis is significantly affected by the location and number of endometriotic lesions.
