目的 探讨采用双侧去骨瓣减压术治疗重型颅脑损伤的临床效果.方法 选取方城县人民医院于2010年12月至2013年12月收治的50例重型颅脑损伤患者并随机分成两组,对照组25例患者采用单侧标准大骨瓣减压术治疗,观察组25例患者采用双侧去骨瓣减压法治疗,比较两组的疗效.结果 对照组治疗前后的GCS评分分别为4.32±0.38和7.27 ±0.61,治疗有效率为72％,其中显效9例,有效9例;观察组治疗前后的GCS评分分别为4.41 ±0.43和8.96±0.39,治疗有效率为84％,其中显效11例,有效10例.观察组治疗情况明显优于对照组.结论 双侧去骨瓣减压术较标准单侧大骨瓣减压在治疗重型颅脑损伤的疗效上有明显的优势,值得临床推广.

Objective To investigate the clinical effect of bilateral decompressive craniectomy on severe traumatic braininjury.Methods Fifty severe head injury patients from December 2010 to December 2013 were selected,they were randomly divided into the control group and the observation group.Twentyfive patients in the control group were treated by unilateral standard hemicraniectomy,25 patients in the observation group were treated by bilateral decompressive craniectomy,the curative effect of the two groups were compared.Results The GCS scores of the control group before and after treatment were 4.32 ±0.38 and 7.27 ±0.61,the effective rate was 72％,including 9 patients with apparent results and 9 patients with effective results; the GCS scores of the observation group before and after treatment were 4.41 ±0.43 and 8.96±0.39,the effective rate was 84％,including 11 patients with apparent results and 10 patients with effective results.The curative effect of observation group were significant higher than that of control group.Conclusions Bilateral decompressive craniectomy in the treatment of patients with severe brain injury has obvious advantages than the standard unilateral hemicraniectomy,so it should be popularized.
