目的 通过检测慢性乙型肝炎患者外周血单个核细胞Toll样受体9(TLR9)及其下游信号分子的表达水平,探讨TLR9及其下游信号分子在慢性乙型肝炎的临床意义.方法 选择慢性乙型肝炎患者22例,健康对照者18例.肝素抗凝血分离外周血单个核细胞,实时荧光定量PCR方法检测外周血单个核细胞TLR9及其下游信号分子MYD88、IRAK1、TRAF6、NF-κB的mRNA的相对表达水平.结果 与健康对照者相比,慢性乙型肝炎患者外周血单个核细胞的TLR9及其下游信号分子MYD88、IRAK1、TRAF6、NF-κB的mRNA表达水平明显降低,分别为健康志愿者的0.298、0.478、0.398、0.421和0.663倍,两组比较差异有统计学意义(P<0.05).结论 TLR9信号通路重要信号分子的表达水平在慢性乙型肝炎患者外周血单个核细胞显著降低,提示TLR9信号通路异常引起免疫紊乱在乙型病毒性肝炎的发病机制中起重要作用,有望成为未来治疗乙型肝炎的重要靶点.

목적 통과검측만성을형간염환자외주혈단개핵세포Toll양수체9(TLR9)급기하유신호분자적표체수평,탐토TLR9급기하유신호분자재만성을형간염적림상의의.방법 선택만성을형간염환자22례,건강대조자18례.간소항응혈분리외주혈단개핵세포,실시형광정량PCR방법검측외주혈단개핵세포TLR9급기하유신호분자MYD88、IRAK1、TRAF6、NF-κB적mRNA적상대표체수평.결과 여건강대조자상비,만성을형간염환자외주혈단개핵세포적TLR9급기하유신호분자MYD88、IRAK1、TRAF6、NF-κB적mRNA표체수평명현강저,분별위건강지원자적0.298、0.478、0.398、0.421화0.663배,량조비교차이유통계학의의(P<0.05).결론 TLR9신호통로중요신호분자적표체수평재만성을형간염환자외주혈단개핵세포현저강저,제시TLR9신호통로이상인기면역문란재을형병독성간염적발병궤제중기중요작용,유망성위미래치료을형간염적중요파점.
Objective Through the detection of toll-like receptors 9 (TLR9) and its downstream signaling molecules expression level of peripheral blood mononuclear cells in chronic hepatitis B patients,to explore the clinical significance of TLR9 and their downstream signaling molecules in chronic hepatitis B.Methods Twenty-two patients with chronic hepatitis B and 18 cases of healthy controls were chose.Peripheral blood mononuclear cells were separated by heparin anticoagulant and real-time PCR was used to detect TLR9 and downstream signaling molecules MYD88,IRAK1,TRAF6,NF-κB mRNA expression levels in peripheral blood mononuclear cells.Results Compared with healthy controls,TLR9 and downstream signaling molecules MYD88,IRAK1,TRAF6,NF-κB mRNA expression levels decreased obviously to 0.298,0.478,0.398,0.478 and 0.663 times in peripheral blood mononuclear cells in patients with chronic hepatitis B (P < 0.05).Conclusions TLR9 signalling pathways and downstream important signal molecule expression levels of chronic hepatitis B patients in peripheral blood mononuclear cells decreased significantly,which prompt TLR9 signal pathway and plays an important role in the pathogenesis of hepatitis B viral hepatitis.And it is expected to become important targets for the treatment of hepatitis B in the future.
