目的 观察白细胞介素(IL)-10在蛛网膜下腔出血(SAH)后的脑血管痉挛(CVS)中的治疗作用及其作用机制.方法 日本大耳白兔(30只)随机分为假手术组(A)、SAH组(B)、SAH+IL-10组(C)、SAH+锌原卟啉(ZnPP)+ IL-10组(D)、SAH+ ZnPP组(E),经枕大池2次注血法建立兔SAH后CVS动物模型,术后C、D、E组分别经腹腔注射给予IL-10、ZnPP,术后第5天采用酶联免疫吸附试验(ELISA)法检测血清中肿瘤坏死因子-α(TNF-α)、IL-6的水平,取基底动脉血管检测血管直径并检测血红素氧合酶-1(HO-1)的表达.结果 SAH后血管痉挛模型造模成功,SAH各组基底动脉直径比A组明显缩小(P<0.05),TNF-α与IL-6含量升高(P<0.05),其中基底动脉直径在C、D组[(733.94±17.28)、(646.11±9.79)μm]较B、E组[(595.64±10.15)、(532.81±17.09) μm]增加,TNF-α和IL-6含量在C组[(26.27±1.64)、(58.15±1.38) ng/L]、D组[(43.45±1.77)、(77.17±1.09) ng/L]较B组[(53.56±1.27)、(115.93±1.47) ng/L]、E组[(60.56±1.79)、(136.45±1.73) ng/L]降低;A组脑基底动脉未检测到HO-1蛋白,C组HO-1含量(0.446±0.019)较B、D、E(0.314±0.014、0.251±0.018、0.160±0.011)组含量增加.结论 IL-10能够缓解SAH后所致CVS,可能是经HO-1蛋白发挥其治疗作用.

목적 관찰백세포개소(IL)-10재주망막하강출혈(SAH)후적뇌혈관경련(CVS)중적치료작용급기작용궤제.방법 일본대이백토(30지)수궤분위가수술조(A)、SAH조(B)、SAH+IL-10조(C)、SAH+자원계람(ZnPP)+ IL-10조(D)、SAH+ ZnPP조(E),경침대지2차주혈법건립토SAH후CVS동물모형,술후C、D、E조분별경복강주사급여IL-10、ZnPP,술후제5천채용매련면역흡부시험(ELISA)법검측혈청중종류배사인자-α(TNF-α)、IL-6적수평,취기저동맥혈관검측혈관직경병검측혈홍소양합매-1(HO-1)적표체.결과 SAH후혈관경련모형조모성공,SAH각조기저동맥직경비A조명현축소(P<0.05),TNF-α여IL-6함량승고(P<0.05),기중기저동맥직경재C、D조[(733.94±17.28)、(646.11±9.79)μm]교B、E조[(595.64±10.15)、(532.81±17.09) μm]증가,TNF-α화IL-6함량재C조[(26.27±1.64)、(58.15±1.38) ng/L]、D조[(43.45±1.77)、(77.17±1.09) ng/L]교B조[(53.56±1.27)、(115.93±1.47) ng/L]、E조[(60.56±1.79)、(136.45±1.73) ng/L]강저;A조뇌기저동맥미검측도HO-1단백,C조HO-1함량(0.446±0.019)교B、D、E(0.314±0.014、0.251±0.018、0.160±0.011)조함량증가.결론 IL-10능구완해SAH후소치CVS,가능시경HO-1단백발휘기치료작용.
Objective To study the effect of interleukin-10 (IL-10) on cerebral vasospasm following subarachnoid hemorrhage (SAH) in rabbit and explore the mechanism.Methods Thirty Japanese rabbits were randomly divided into Sham operation group (A),SAH group (B),SAH + IL-10 group (C),SAH + Zinc protoporphyrin (ZnPP) + IL-10 group (D) and SAH + ZnPP group (E).The animal model of cerebral vasospasmfollowingsubarachnoid hemorrhage was made by infusing autologous artery blood twice into the cisterna magna.The C,D,E group were respectively received IL-10,ZnPP through intraperitoneal injection after the SAH model was established.The tumor necrosis factor (TNF)-α and IL-6 levels in serum were detected by enzyme linked immunosorbent assay (ELISA) method 5 days postoperatively.The diameter and expression of heme oxygenase-1 (HO-1) protein in basilar artery were observed.Results The animal model of cerebral vasospasm after subarachnoid hemorrhage was success.The basilar artery diameter of SAH groups was significantly shorter than the sham operation group (P < 0.05) and the TNF-α and IL-6 levels were higher in SAH group (P < 0.05),The diameter of group C and D [(733.94 ± 17.28),(646.11 ± 9.79) μm] was increased than group B and E [(595.64 ± 10.15),(532.81±17.09) μm],the TNF-α and IL-6 levels in group C [(26.27 ±1.64),(58.15 ±1.38) ng/L] and D [(43.45 ± 1.77),(77.17 ± 1.09) ng/L] were lower than group B [(53.56 ± 1.27),(115.93 ± 1.47) ng/L] and E [(60.56 ± 1.79),(136.45 ± 1.73) ng/L].There wasn' t the expression of protein HO-1 in sham operation group and the content of HO-1 in C group (0.446 ± 0.019) was higher than group B,D,E (0.314 ± 0.014,0.251 ± 0.018,0.160 ± 0.011).Conclusion IL-10 can alleviate the cerebral vasospasm caused by subarachnoid hemorrhage and may be mediated by the HO-1 protein.
