目的 研究CD40及基质金属蛋白酶(MMPs)在颈动脉易损斑块及稳定斑块中的表达,探讨CD40与MMPs表达的相关性及影响斑块稳定性的可能机制.方法 收集颈动脉内膜剥脱术(CEA)后的粥样硬化斑块标本32例次,根据术前颈动脉超声、经颅多普勒(TCD)及是否发生急性缺血性脑卒中事件分为易损斑块组及稳定斑块组,采用PCR和Western blot法检测两组标本中CD40及MMPs的mRNA和蛋白表达水平.结果 易损斑块组CD40和MMP-9 mRNA及蛋白表达水平均较稳定斑块组显著增高(P<0.01);MMP-2蛋白表达水平亦较稳定斑块组明显增高(P <0.01);CD40mRNA与MMP-2 mRNA(r=0.43)、CD40 mRNA与MMP-9 mRNA(r=0.90)之间呈正相关(P<0.05).结论 CD40不但参与动脉粥样硬化斑块的形成,而且可能通过调控MMP-2、MMP-9的表达影响动脉粥样硬化斑块的稳定性.

Objective To study the expression level of CD40 and matrix metalloproteinase (MMPs) in vulnerability,stability carotid atherosclerotic plaque.Investigate the relevance of CD40 and MMPs expression and the impact on plaque stability.Methods Thirty-two cases of specimens with carotid atherosclerotic plaque were collected from carotid endarterectomy (CEA).According to preoperative carotid ultrasound,transcranial doppler (TCD) and the presence of acute ischemic stroke events,all specimens were divided into stable plaque group and vulnerable plaque group,detected the mRNA and protein expression level of CD40 and MMPs in each group using PCR and western blotting assay.Results The mRNA and protein expression level of CD40 and MMP-9 in vulnerable plaque group was significantly higher than in stable plaque group respectively (P < 0.01).The MMP-2 protein expression level in vulnerable plaque group was distinctly higher than in stable plaque group(P <0.01).There were positive correlation between CD40 mRNA and MMP-2 mRNA (r =0.43,P < 0.05),CD40 mRNA and MMP-9 mRNA (r =0.90,P <0.01) respectively.Conclusion CD40 is not only involved in the formation of atherosclerotic plaque,but also may influence the stability of atherosclerotic plaques by regulating the expression of MMP-2 and MMP-9.
