目的观察高压氧(HBO)综合治疗溃疡性结肠炎的临床疗效及抗复发情况,探讨HBO抗溃疡性结肠炎复发的可能作用机制.方法将70例溃疡性结肠炎患者按治疗方法分为2组:治疗组36例和对照组34例,治疗组HBO联合柳氮磺胺吡啶(SASP)治疗,对照组单用SASP治疗,2组均治疗45 d.结果治疗组总有效率为94.44％,高于对照组的67.64％,复发率为11.76％,低于对照组的39.13％,差异有统计学意义(P＜0.05,P＜0.01).结论 HBO联合SASP治疗溃疡性结肠炎疗效好,复发率低,其可能机制为:通过抑制T淋巴细胞亚群的过度增殖,改善T淋巴细胞亚群的比例失调,调衡细胞免疫状态,达到抑制溃疡性结肠炎复发的目的.

Objective To observe the clinical effects of hyperbaric oxygen (HBO) comprehensive treatment on ulcerative colitis and resistance to recurrence and to explore possible mechanism involved in the recurrence of the disease.Methods Seventy cases of ulcerative colitis were randomly divided into 2 groups:the treatment group (n =36) and the control group (n =34).The treatment group was treated with HBO combined with sulfasalazine (SASP) for 45 days,while the control group was only treated with SASP for the same length of time.Results The therapeutic effects on the treatment group were superior to those of the control group,and the recurrence rate was lower than that of the control group,with statistical difference (P ＜ 0.05,P ＜0.01).Conclusions HBO combined with SASP produced good effects on ulcerative colitis,with low recurrence rate.The possible mechanism might be associated with the inhibition of excessive T-lymphocyte subgroup proliferation,improvement of T lymphocyte subgroup imbalance and adjustment of cell immune status,thus suppressing the recurrence of ulcerative colitis.
